What kind of doctor treats small fiber neuropathy? Neurologist? Rheumatologist? Other?

@jansendscards, typically neuropathy is comes under that care of a neurologist. Treatment depends on the underlying condition. For example, small fiber neuropathy caused by prediabetes or diabetes is treated by managing blood sugar levels and maintaining a healthy weight, in which case you may be under the care of an endocrinologist.

When the cause of SFN hasn’t been identified, treatment focuses on managing symptoms.

Do you know the underlying cause?

@jansendscards I went to a neurologist for the SFN diagnosis. From what I have learned, SFN is best diagnosed with a skin biopsy. The diagnosis is probably the easy part. Determining the root cause can be more challenging. The most common cause is diabetes. From what I have read, they are even finding SFN in patients that are "pre-diabetic" but not diagnosed diabetic yet.

Treating SFN primarily begins with treating the root cause (if possible). Beyond that, we treat symptoms. I have been chasing a treatment for my SFN for years. I have tried duloxetine but that did not work. I found that pregabalin and oxcarbazepine worked well. Pregabalin worked better for me than gabapentin.

I am T2 diabetic and have found that whenever I indulge in high-carb foods, my SFN symptoms seem to flare. I cannot find any medical confirmation of this but it does seem to happen for me.

Finding a medication or medications that help you is a bit like chasing the wind, but don't give up. You are your own best health advocate. Continue engaging your primary care doc and also reaching out to boards like this.

I am not a doctor and this is not medical advice. Please work with your physicians and I wish you the best.

My SNF has been verified by a biopsy. Yesterday with rain on its way every fiber in my body was screaming at me. I am allergic to so many medicines that even the common ones I can’t take. I also have fibromyalgia. My rheumatologist does not know what else to do. Can a neurologist help?

@jansendscards, Have you thought about trying any complementary or integrative therapies? The Foundation for Peripheral Neuropathy has some listed here - https://www.foundationforpn.org/treatments/

Many people have found that Myofascial Release Therapy (MFR) helps. There is a discussion on MFR that you might find helpful also.

Have you heard of or tried MFR?

I'm pretty sure MFR can be done by a qualified therapist trained in providing Myofascial Release Therapy. If you go to the discussion and watch the John Barnes video in the first post it does a good job of explaining how it can help. Have you tried any stretching exercises for the feet?

I'm not sure there is anything that provide immediate relief for the numbness in the feet. I've liked the small handheld portable massage devices but they really didn't provide much relief and what it did provide didn't last long for me. I do think it really helps to keep your feet moisturized daily with some kind of good skin lotion.

I have had this for 11 years. Nothing seems to help except for low dose hydrocodone.. I only take two a day, 5-325 strength and they help pretty much. However, lately the pain is more severe (the weather maybe?) and my feet are freezing at night, so electric blanket for a couple of hours helps some. The last two weeks I have woke up with leg or feet cramps, and the Caleb Treeze Organic Farms Stops Leg & Foot Cramps helps stop those. (Amazon) With the new year, I am joining all the others who are out of shape and I will start exercising a bit. That helps.
