Blood in Semen after Prostate Biopsy

I couldn't find an existing thread on this topic, but I wanted to share what I'm seeing so far, and compare with other's results.

I'm 58 years old. After discovering a 5+ PSA result in a routine adult physical, I was scheduled for and received both a MRI, and a Transperineal biopsy. In my case, they took 16 samples. I do not yet have the pathology results.

I was told there could possibly be some blood in my urine, but to watch for clotting, etc and large amounts. I was also told there would likely be some blood in my ejaculate, tapering off over time. I understand the amount of blood can be influenced by number and location of samples.

I was not prepared for what happened though. Four days after the biopsy, and to my horror, I found my expectations were way underestimated. It was not "streaks or spots" of blood, nor was it red-tinted. It looked to be pure blood. The volume was probably double what I would have expected. Hoping that the next day would be more normal, I again tested the waters.. It was nearly as bad, however, the color seemed to be a cross between pure blood and fresh motor oil.

I've got a message into my care team, and I'm awaiting a response while I also await pathology. I'm a bit nervous.

Has this been other's experience? If it is common, why understate it in the publications I was provided?

I have not noticed blood in my urine, however, it tends to be dark-ish as I tend to under-hydrate.

@ Post- biopsy it's quite. common.. I'm quite suprised that the literature did not mention it! After my biopsy ( my PSA wuz' @ 7) the Urologist made it a point to mention that I might see "tainted" urine for awhile & that it's normal . Don't worry !

I do not understand the reason for the biopsy as a 5+ PSA for a 58 year old is not a trigger by itself?

Thanks for the quick reply. While I mentioned urine, the focus of my concern was ejaculate (semen). I hope that clears things up.

I experienced the same. It’s very common. I called the urologist several times. They were very helpful. They explained a very small niche could result in what appears to be a lot of blood. Since I don’t like seeing blood especially my own, I closed my eyes or sat down when urinating. Call your urologist if you’re worried. They are there to help. Wishing you all the best!

To clarify, it was two consecutive 5+ PSA results a few weeks apart. The MRI also showed a PI Rads-3 lesion and prostatitis.

My experience was exactly the same. Semen and urine. Looked like pure blood. It evidently isn’t something to worry about. It took a little time, but completely resolved on its own.

Had a biopsy 8-18-22 and 15 samples, how long should I expect blood when I have sex?

I think you'd need a lot more medical "history" to answer that one.

For instance after my biopsy, there was some blood until my surgical removal a couple months later. I'd certainly make sure my doctors were aware. There's too many variables to get that answered with that much information, there may be other things at play here.

Hi Murray, I agree with @web265. That might be a question for you doctor.

I hope the biopsy was negative. Are you still experiencing blood with sex?

It's been almost two weeks since my TP biopsy. 12 cores. Looks like pure blood to me too. I'm not getting blood in my urine now, though I did at first. I saw a few small clots in my ejaculate but now it sure enough looks like pure blood. I've read to expect that maybe for a few months. I have a Zoom meeting with the doc on 10/5. I'll ask him about it and post again if he thinks there is any concern. My wife told me that if I were a woman I wouldn't be too concerned about some blood "down there." Good luck with your path results.
