Hi @leesagalow7, welcome to Mayo Clinic Connect. Can you tell me a bit more? How did this pain start or what is the cause?

oh my, thank you for sharing this, I am embarassed to share this sometimes. I have intermittent rectum pain due to a huge hemmoroid that I had after my gave birth to my 9.5lb daughter. It's only painful when I have gas, it's sharp pain. I'm so sorry that you have this, I have it intermittently, its awful.
How do you releave the pain?
I typically have a sharp pain in my rectum with gas. But I cannot feel that I have gas. To relieve the pain, I sit on the toilet and 'strain' a bit, and pass a little gas bubble, a tiny fart. The sharp pain in my rectum for me is usually at night, and wakes me up. I picture the pain as perhaps a fold of some kind in my rectum. I had to go back to the doctor after the baby was born, reopen my episiotomy and resew it back up. Hugest hemmorroid he'd ever seen he said, and he showed all of the staff.
@leesagalow7 I'd like to extend my welcome. You are in constant unbearable pain, have been to pain centers, and tried a nerve block but nothing has helped.
Below I have linked a short conversation related to the topic. You may want to read through the previous suggestions.

I understand that you visited other pain centers but may I ask if you received a second opinion were diagnostics were ran?
@leesagalow7 I just thought of something, this brings back memories. When I had my 9.5lb baby, I had a vein push through my episiotomy incision. I was in excrusiating pain. I thought I should add to my story, and that vein pushing through my episiotomy was worse than having the baby itself! Perhaps you have a vein in your rectum, poking through an incision. Mine pushed through because I was walking around too much after, and I was not to walk up stairs. Perhaps talk to your doctors, poke a camera up in there, look around, there could be a vein pushed through your incision of your previous surgery. Get a 2nd opinion, take a look. My excrutiating pain went away after the physician lanced the vein and pushed it back inside (his description). good luck to you, I hope my story helps you, I wish I could do more.
I can empathize. I was misdiagnosed with hemorroids many years ago. The pain was unbearable....like giving birth each and every time I had to pass a stool. I dreaded it. I got a second opinion to discover I had a rectal fissure. After a short surgery to repair the fissure I have not had the problem again. I urge you get a second opinion and I pray you find proper relief.
I had surgury about 5 years ago I now have very poor blood circulation and cannot go under anesteasur {spelt wrong) I wonder if area can be numbed
Thank you I will speak to my doctor
