no, he hasn't seen a doctor. he thinks it's unimportant since "he's still alive" and it's been happening for years. but i'm curious to know what it is

Agree it is symmetrical and grid shaped.
It appears to be externally caused by contact with
something manmade in his environment interacting with skin
such as clothing , sporting equipment, furniture, heating pad, pressure,etc. A dermatologist might do patch tests if it appeared inflammatory.
