Decision time SBRT, IMRT or surgery

Posted by harvey33 @harvey33, Jul 28 8:31am

diagnosed 1 1/2 months ago biopsy shows grade group 3 with 8 of the 15 being positive and Gleason of 7. I have been offered SBRT, IMRT, and surgery. I am leaning toward the SBRT. I am just very confused with all the information I have read about the short and long term side effects. I also have an abdominal aortic aneurysm that last measured was not very large and mild COPD and am 69 years old in decent health otherwise.

I need to go forward with this decision just looking for real life experience with all 3 choices.

Thanks in advance

Good morning!

I had my first session of SBRT 1 of 5, yesterday. OF ALL OF THE THINGS I'VE DONE SO FAR this was by far the easiest. Had an empty rectum, drank 16 oz of water an hour before. Name called, changed my clothes, laid down on the table with the mold for my legs, they lined me up, asked me what kind of music I wanted to listen to (of course I told them classic 70's), laid still for 10 minutes, they said you're done, changed my clothes and left.

I guess what I'm saying is the finish line (at least for now) is in sight. I'll post if there are any side effects which I'm going to be optimistic that they will be non-existent or minimal at best. I just wanted to share that for me this was a non-event as far as a treatment.

Good luck to everyone and stay positive!

@sorensend58 Thanks. I feel sort of like you once I get the first one done 11-12 I will be on the downward slide other than the waiting and wondering if I really made the correct treatment decision.

I was diagnosed in January of 2020 when I was 71. Had a gleason score of 9. I opted for IMRT, brachytherapy, and Lupron (for 12 mos). My psa went undetectable for almost 5 years until July of 2025 when the cancer spread to my lungs. For me, I felt it was the right decision and it gave me 5 years of remission with few side effects and a high quality of life. I had a lung resection in Sept of 2025 to remove part of my left lung, which is where the worst nodules were. I then went on Abiraterone, Pregnisone, and Lupron (again) and so far so good, with almost no side effects.
