Decision time SBRT, IMRT or surgery

Posted by harvey33 @harvey33, Jul 28 8:31am

diagnosed 1 1/2 months ago biopsy shows grade group 3 with 8 of the 15 being positive and Gleason of 7. I have been offered SBRT, IMRT, and surgery. I am leaning toward the SBRT. I am just very confused with all the information I have read about the short and long term side effects. I also have an abdominal aortic aneurysm that last measured was not very large and mild COPD and am 69 years old in decent health otherwise.

I need to go forward with this decision just looking for real life experience with all 3 choices.

Thanks in advance

@harvey33 I have heard lots of good things about SBRT. I myself am still not sure if I should take the radiation route or RARP...it seems like the radiation SBRT treatment is a somewhat shortish journey and you will be good as new in no time!

Well the Good as New is questionable but is the route I have chosen. I’m 69 and the surgery just had more side effect possibility’s and the minus of having surgery if cancer comes back. Good luck on your decisions and I have been away for a while but have been busy with business and research.

These decisions are tough and I wouldn't presume to make a recommendstion . In my case, I opted for IMRT rather than SBRT. Briefly considered surgery but I'm in my late 70s, so decided against it.

Doctors gave me the usual info that the results are similar. I did not want the number of treatments to influence my decision.

So Gleason 9, barely intermediate Decipher score (.58) and high PSA (25) led me to my decision. While my PSMA PET scan was clean, no test is 100% accurate. IMRT gives some radiation to surrounding areas. Also on Lupron.

I finished two weeks ago. I don't assume I will be 'good as new'. Will see if my first PSA test in January looks good. I was told it will take a year to judge the success of the treatments. So you get to sweat PSA tests every 3 months for a year or two.

@tbgb50 I was Gleason 9 had ADT andIMRT. I am still having PSA tests every 5 months six years later. PSA on most recent test was undetectable.

Congratulations! I have my post radiation treatment appointment with the RO on Monday. It's nice to hear you a great results as I transition from worrying about the treatments to worrying about their success.

@harvey33 Are you on a 6 month course of ADT? Is it Orgovyx? Are you doing anything to mitigate the side effects?
