No explanation yet for significant rising PSA

Posted by shelby29 @shelby29, Oct 15 9:34pm

Gleason 4+4, stage T3b. 28 radiation and 3 years Lupron injections. 1 year later, PSA rose from zippo to 2.1 in 3 months.
PSMA Pet scan result, no uptake, Prostate specific MRI, no cancer.....now what??
Doc will repeat PSA, but what could cause PSA rise??? Any thoughts....

2 questions, is Orgovyx a substitute for Lupron?
What do you mean by radiation options? I'm confused. Isn't ADT+ARPI suffient?
Will let you know what is decided on Monday.
We really like our PA and she obviously cares about us, so we trust her for now! Thank you.

Orgovyx is a pill you take once a day that replace Lupron,

Because they can’t find anything it’s hard to do anything but drugs.

I was thinking of somebody else’s message when I replied with those radiation types. I suspect the drugs would be really the one thing they would do right now.

@shelby29 Interesting —> His testosterone was suppressed but not his PSA. Is he hormone resistant? If so, that might result in a different path.

REPLY

Yes, no testosterone but PSA is 2.1. Thought process is androgen gland or cancer cells producing prostate androgen.

@shelby29 If he has been on ADT since January 2023, he may have become castrate resistant. Typically, an ARPI is added to see if that restores control.

Yes, we see doc tomorrow. Thanks

We saw our oncologist today. They retested his PSA and it came back negliable. Also did a urine test which came back positive for blood. That may be the reason for the rise in PSA 4 weeks ago. Both test will be run again in 4 weeks. Doc wants to test PSA monthly for awhile....just to be sure.
If anything changes they will move care to mayo rochester oncologist specializing in advanced prostate cancer. Thankful for the positive news today!!
