Steady PSA no uptake on PSMA SCAN

Posted by firespooks @firespooks, Oct 3 4:27am

I had a Prostatectomy in 2006 then Salvage radiation in 2013 , for a recurrence. My PSA has slowly risen until 2021 . It has stayed around 1.12 up and down slightly for the last several years. I just had a PSMA and it didn’t show any uptake. What does all this mean ? Im not on any medication at this time.

The fact that your PSA isn’t rising is a good thing, Regular prostate cancer is apparently not growing and spreading.

The problem is that after a long time, the prostate cancer can change and become neuroendocrine prostate cancer. That cancer doesn’t produce PSA, which could be why your PSA isn’t rising. Neuroendocrine prostate cancer does not produce PSMA so it can’t be found on the scan. You need to do an FDG PET scan to find it since it consumes glucose which regular prostate cancer does not do.

It could just be that your cancer isn’t active anymore, Or is growing so slowly that it can’t be seen in the scan.

I had a prostatectomy in 2010 and salvage radiation in 2014. After all these years, I worry about having it change to neuroendocrine.

Lots of options to talk to your doctor about.

Wouldn’t that be pretty rare without ADT?

Not sure quite what you mean.

Neuroendocrine prostate cancer Can occur with or without ADT.

If somebody’s PSA rises a little bit and doesn’t rise significantly for a long time, it could just be that They are in remission.

Ok. I thought NE generally developed after a long course of ADT. I thought it was pretty rare.

I have a PSA 7-9 for the last 4 years.
And also have a extremely large large prostate. 73 years old, no symptoms at all ( happy)

A friend of ours had a huge prostate and his PSA was always around 50. He had multiple biopsies and never found anything. He died of a completely different type of cancer.

Sounds like you’re doing just fine.
