Would you go ahead with a Prostate removal?

Long story... 69 year old Male
I've had a enlarged Prostate for a few years....PSA has been as high 16's and as low as 5.27 on finasteride.
3 biopies the last one MRI-computer guided they only found a couple small lesions but no cancer... that was 2 years ago
July this year in the middle of the night..I couldn't pee... went to the ER
they did the optilume processor and catheterized me and sent me home...3 days later they removed the catheter and that night I stopped peeing again....Back to the ER..back in went the catheter..this time 7 days in.... 2 day after removal I couldn't pee back to the ER
and I went to see my Doc within a week..They taught me to self catheter until I could see the surgeon ... I catheterized a couple times and then I just said nope..lets wait and see and it was fine, no problems for a 10 days...Saw the Surgeon he set up my next appointment for Oct 7th..for all the test...
He does the robotic..removal through your penis Yikes...lol
So I've only had to catheterized one time in the last 3 weeks but I do get blood once in a while and I think maybe it was a clot that caused that...
My question is this.... Since everything seems to be working and if it does stop I know how to self-catheterize ...Should I go to the extreme and remove the prostate..if they don't find cancer on the 7th..I mean I don't know if they are looking or doing a MRI or ?..
I just feel like maybe I'm going to far and maybe the end results will be worse then the present problem..which other than maybe stop peeing, there isn't nothing new.
One other kicker.........I've hit my Out of Pocket max with my INS..so this would be 100% paid......LOL jeeezzz

No. Why risk incontinence and ED. Have all your male relatives had serious cases of prostate cancer? That may be a consideration. I assume you have already been on max doses of Flowmax or the like and they are no longer effective. There is much less chance of negative side effects from shrinking the prostate by hollowing it out vs. RP. There is Aquablation, Tulsa Pro (HIFU through the penis, used for focal prostate cancer treatment) among other procedures used for BHP. All will reduce prostate size and shrink it with less risk.

You could have them do a Turp Which is like a rotor router of the prostate. As long as you don’t have cancer, that could take care of the problem. This is something you need to speak to your urologist about.

Robotic removal of the prostate through the penis would be a new technique all right. I don’t suspect this is really a complete removal.

Have you been to a urologist to discuss what’s going on? That should be the first place to go rather than a surgeon.

I think your right... Its hollowing out the prostate.. They did mention that the urea tube going through the prostate would be removed and they then sew the the two ends together... Not sure which process that is...I'll know more on the 7th
I'm just looking at the pro's and cons of this, from people who "Know" or have taking the path before me...
I'm still on the fence...I'd say 50/50 because I feel fine...and I feel good knowing I can take care of the problem if I stop peeing..
Also..I'm not on Flomax... don't really need it and when I did take it, it would make me dribble a little something..giving me less control. of course this was before this peeing problem started in July

Hi, this is a decision that only you can decide to make. I wouldn't do it if cancer has not been detected. I had my prostate removed because a biopsy determined I had cancer. My brother in low, 91 years old now, has been dealing with the issue you are dealing with since he was 78. Medical treatments have kept him functioning well without going to the extreme of prostatectomy and be subjected to the unpleasant side effects.
Good luck

@jeffmarc If you keep going to a surgeon, you will probably end up with surgery lol. That's what they do. As Jeff stated, perhaps try a Urologist where they will test then talk about All options.

Why? Second opinions are Invaluable! From my understanding of things and your description a TURP procedure may be a better option. It is worth finding a center of excellence and another opinion or two.

Aquablation worked well for me and eliminated all my urinary systems and pathology on sampled tissue from the procedure was benign, but a year later my PSA increased to 15 and I've been diagnosed with PC.

@casd57 There are many procedures to help you if no cancer is found.
It sounds like you have a tendency to become blocked so this ‘quiet period’ probably won’t last…sorry!
TURP has been mentioned, along with aquablation; there are also laser surgeries like HoLep which are very successful, and they can biopsy the tissue at the same time, so there is no need for a separate procedure. Best,
Phil
