Tymlos vs Evenity

Hi!
I’m new here and am happy to have found your page!
Trying to decide between Tymlos and Evenity.
68 years old – 5’2” and 98 lbs – very active with exercise classes and walking - I just had my second back surgery (clean-out on right side in 2021, and this one was a fusion at L-4-5 – doing well!) – have a right hip replacement – family history of osteoporosis – hysterectomy at 48 years old with no HRT – have done Reclast three times – my doctor won’t repeat Reclast – I have low blood pressure and have to get up slowly at times if I get dizzy.
T-Scores are:
Spine: -1.5
Femoral neck: -3.2
Hip: -2
My husband is 86 and has dementia; however, we still travel frequently internationally for up to two-plus weeks at a time. I am his sole caregiver.
My endocrinologist recommends Tymlos. I usually go with what my doctors recommend, but given my living/travel situation, I’m not sure it will be smooth sailing for two years with daily Tymlos injections.
I'd sure appreciate any input to help me decide between Tymlos and Evenity.

Evenity is easier, but does require monthly injections at the doctor's office for 12 months. Tymlos does have a side effect in some people of lowering blood pressure temporarily after the injection; maybe that would be a problem for you?

Plus you have to order Tymlos at the same time every month, from a specialty pharmacy, and it gets delivered to your house. There's some wiggle room with the delivery date, but if you're away for two weeks, it could be an issue.

It seems like 12 injections, which you can schedule, may work better for you.

@njx58 I didn’t realize about the home delivery! Thank you so much for your thoughts!

The strongest and longer term chance of building bone on anabolic, before locking-in gains, would be 2 years of Tymlos-->followed by Evenity-->followed by antiresorptive (i.e. Reclast). If you reverse Evenity --> Tymlos, the Evenity will blunt the effectiveness of the Tymlos (b/c it has an anti-resorptive component). The Tymlos pen does not require refrigeration after its first use and the manufacturer will provide a nifty carry case (and an injection site charting map). Worth a good conversation with your doc to hear all pros & cons AND to ask what his plans are post-Tymlos.

I had no clue about this: If you reverse Evenity --> Tymlos, the Evenity will blunt the effectiveness of the Tymlos (b/c it has an anti-resorptive component).

Thank you SO MUCH!

Hi! I have had blood pressure on the low side all my life and have been taking Tymlos for about 2 weeks now. I have been experiencing a lot of side effects that have gradually changed over the last two weeks. After my first injection, I got an elevated heart rate for about 30-40 minutes. After a few injections, I got terrible fatigue/very low blood pressure about 3-4 hours after the injection where I had to take a nap/lie down. This week has been some mild nausea and diarrhea, but no fatigue. Through research on this site and others' experience, this is very common in the first few weeks as your body get used to the medicine. If you decide on Tymlos, you may want to not have any upcoming travel plans as you get used to how to take the injection and if you have any side effects. For reference, I am 56 yr old postmenopausal female with -2.0 spine and -2.3 hip with two fragility fractures in the last 3 years (one was a major wrist fracture/shattered) and I will be having a L4-5 fusion after my density gets back up with Tymlos. Good luck on your journey!

So much great information. Sorry you had the initial side effects, but I really like your idea of not traveling for the first few weeks.

I have to after October 24 (one year after my last Reclast), and our next trip will be November 28, so that should give me time to adjust as needed.

I think I'm leaning towards taking the injection at night before bed, so some of the symptoms you mention might be minimized.

Your reply is much appreciated. Best of luck to you!

@shbo , if you choose Tymlos, you can titrate up to full dose, and give your body time to adjust. I got a bad reaction to the first full dose, so I started over with just doing 1 click for a few days, then 2 clicks for a few, 3, ... etc. I personally can't handle more than 5 clicks (of the 8 in a full dose), and I've been on it for 11 months. My Dr agrees to let me stay at 5 clicks, and I'm getting great gains, even at partial dose. If I travel domestically, I'll take it with me, but internationally I usually just take a break, and start back at a lower dose when I get back, work my way up again. If I was to go for a one or 2 month trip internationally, I would probably consider taking it.

I agree with @cathyf31 about the order- she nailed it. Insurance companies don't like to cover the builder drugs like Tymlos because they are more expensive, so you need to fight for it. Read and learn as much as possible in this forum, there are threads for everything.

@boneoptimist
It's so helpful to know you can stop for a bit while traveling. Of course, I'll run it by my doctor.

Can you start with a new pen before traveling to get that day-one refrigerated situation out of the way, then, while traveling, go back to the original pen to use the rest of it, and then finish up with the new pen?

Hope that makes sense!!!

Hello, I'm leaving today for 7 weeks in France. I also spent a month there in the summer. I traveled with my Tymlos. There is no reason to miss a day of medication. I purchased a travel cooler like a thermos that keeps the medicine cold for up to 30 hours. I also brought the box with the prescription on it. I was asked to see the prescription in London, but otherwise no worries getting through security. The one thing that makes traveling difficult is during a hot summer. The medicine should not get over 78 degrees. The thermos has an inner piece that needs to be frozen. If staying in a airbnb it's easy, not so much a hotel. I have a home in France so I just keep it in the fridge all the time. The same at home. I'm going over to Spain mid October for 2 weeks and I will just carry it as temperatures will be perfect by then. I'm having great results and don't want to miss a day of bone growth!
