Anyone experiencing stricture after radiation treatment?

Posted by mike72 @mike72, Sep 6 6:52am

was diagnosed with Gleason 3+4=7 prostate cancer in 2021 and received radiation therapy (EBRT/VMAT, 6000 cGy in 20 fractions) combined with two years of ADT. Five years after treatment I developed a dense bulbar urethral stricture — had VIU procedure .
Comments welcome on similar outcome

According to what I have read the most effective and durable treatment for a dense, radiation-induced bulbar urethral stricture is open surgical reconstruction, known as a urethroplasty

Has any doctor discussed this with you?

I had a VIU
then cutting the scar tissue open at 12 o’clock under direct visualisation while watching on a monitor throughout. ✅
Getting checked in 3 months to see if stricture is gone... hoping for no occurrence but it is a side affect of the radiation as confirmed by my oncologist

@jeffmarc I had urethroplasty. Needed 13 months with suprapubic catheter to heal everything after radiation damaged it all and hyperbaric. After healed I got my AMS800. Works pretty good.

I got a Stricture just below bladder neck. Combined with hemorrhagic bladder cystitis caused blood clots in the bladder causing urinary retention- an absolute horror show lasting 9 months. But I digress. Back to the stricture. the Surgeon thought the odds with a urethroplasty was 60/40 due to the length and location. Failure would make a salvage operation more dicey. I read the VIU and a balloon insertion both have a history of relapsing. I opted for a balloon procedure. It’s the most benign. I’m awestruck there has been no relapse after 4 years.

FYI: Anyone thinking about a balloon should inquire about the Optilume balloon, which coats the urethra with a chemical agent to make it more enduring.

@mike72 Mt. Sinai, Miami Beach. Dr. Billy Cordon. It’s fairly widely known and practiced, and not very complicated. I think Guys at Cleveland Clinic near Ft. Lauderdale might be doing it also.
