Elevated Liver Enzymes When Taking Exemestane

I have been taking Exemestane every other day since January 26, 2026. 3/31/26 my Alkaline Phosphatase spiked over 900. Onco ordering CT-Scan and more in-depth bloodwork.

Anyone experience this who is taking Exemestane or otherwise?

Does anyone have updates on their situation? I have been taking Anastrozole for almost 4 and 1/2 years for stage 1 IDC and my liver enzymes started going up in July. Right now the numbers are in the 50s but I’ve had an ultrasound, MRCP and extended blood tests. Everything is normal. I’m having an endoscopy next week. I have not stopped Anastrozole yet. I have another CBC blood test in 3 weeks and will decide then.

Does anyone have updates on their situation? I have been taking Anastrozole for almost 4 and 1/2 years for stage 1 IDC and my liver enzymes started going up in July. I had extreme liver spike almost in liver failure from Exemestane. Obviously went off of it. Just started Tamoxifen.

O my goodness. Another side effect of Exemestane I was not aware of.

I started it March 30, 2026. My last blood work has been good but I will double check the last time I had my liver enzymes checked and results.

Thank you for bringing this to our attention for those of us on Exemestane.

Hi! I have been on Exemestane since march of 26. No side effects to speak of, thank God. My bloodwork is normal,ut I take Propel a water supplement with electrolytes in it. No problems with blood. Switch and then do blood work.

Been on exemestane for a little over a year and no problems with liver values, all still in the normal range. However, I have had increased kidney values last 2 samples starting about 7 months into the exemestane. Just watching for now and seems stable and even a little reduced. However, may have to switch to letrozole soon as I can’t go without sleep and that’s a major side effect for me. Will have to see if all normalizes once off the exemestane.

Hi! Watch switching too quickly . Did you try Propel that we talked about before. Also, talk with your oncologist about taking exemestane every other day. Other are doing this instead of switching to Letrozole. Propel is a water supplement, and I have no side effects from Exemestane. Water supplement with electrolytes. Clear with you doctors. But, my oncologist was ok with this and highly recommended this.

Initially my Alk Phos was in the too high range and now it's considered too low, I've been on exemestane for 3 years. Both my liver and kidney values are always way off since being on the treatments for my breast cancer. As lesliek mentioned, my dr never seems to care, even when the values seem very bad. They only ever care when my ANC goes to 0.8 or lower, nothing else ever gets mentioned. My PCP gets concerned about all of it but oncology never cares.

Switch oncologists! Too many things off and Cancer doctor doesn't care??? PCP, is on to something. Follow that!!
That's part of their job to care about you?? If oncologist isn't concerned, I recommend docs switching cause your trust value is low!!!

@angele2times did go over every other day dosing on exemestane and he won’t approve it. So if that was my choice, I would do it against medical advice. I was off the exemestane for 6 weeks and was 80% improved. Once restarted, side effects started again within 2 weeks. I do electrolytes but not as a sole source of fluid intake. He took me off exemestane “till I felt better” and switched to the letrozole. So hopefully not doing this too fast I stuck with the exemestane got 18 months. I’ll look into the propel to see if it’s any different from what I use. At least I wouldn’t have to mix it. Appreciate the input.

I personally feel the Letrozole is over rated. But, you want comfort, and every body, to feel comfortable. But, I do know that you can to exemestane everywhere day. With same results. Good luck, try Propel it tames the beast of side effects for me .
