Chronic Constipation and Other GI Issues

Posted by vincescs @vincescs, Aug 24 12:20pm

I've been struggling with GI issues that have gotten progressively worse over the last 10 years including GERD, Barrett's Esophagus, diverticulosis, chronic constipation, and unintentional weight loss. I tested negative for Crohn's, Celiac, Ulcerative Colitis, etc. but GERD and motility issues are ruining my life.

Currently taking esomeprazole, motegrity, MiraLAX, Senokot, Citrucel, and digestive enzymes. I've experimented with lactose-free, gluten-free, and low FODMAP diets with no improvement. Eating more fiber just makes things worse.

I've had a recent CT scan of my abdomen and a recent colonoscopy. No root cause issues were identified. My current GI doctor is not interested in any other testing like SIBO, pH monitoring, colonic transit, or secretin pancreatic function (PFT). I have an appointment scheduled at a Digestive Disease Center that has a Motility Disorders Program, but that is more than six months away.

I've looked into some Integrative Medicine providers in my area, but none take health insurance, and most operate using a membership or subscription-based approach. Many of those providers also sell their own supplements. I'm a bit skeptical of this approach and it feels like a patient grasping at straws for help might spend a lot of money chasing a solution in this space.

If anyone has any other ideas for me, please feel free to share. Open to experimenting with other meds, supplements, diets, etc. Also, if you had a positive experience with a GI doc or Integrative Medicine provider in Philadelphia or the surrounding suburbs please share.

Thanks!

I'm so sorry to hear you are experiencing all these GI issues. My son, who is 28 now, dealt with chronic GI issues most of his life. He has a peptic ulcer, eosinophilic esophagitis, pancreatic insufficiency, GERD, chronic constipation, gastritis, and an issue that they notated was " due to a previous viral insult" but they could never say more than that. We did endoscopes every year. He was on a gluten, casein, dairy, wheat, soy, and corn free diet for 7 years. That did help with the symptoms. He took many different medications for the reflux, pancreatic insufficiency (enzymes), constipation, and the ulcer. So many that I can't name they all. We did elevate his bed at night to try and help with the reflux. Anyway, long story short, after keeping him on the diet for 7 years, his endoscope came back clear two years in a row. We reintroduced the foods during that time and no issues. However, he does still get reflux and is on pepcid daily. The chronic constipation was the worst. He could not use the bathroom without a glycerin suppository, even though he was on Miralax and extra fiber. We found a new gastroenterologist. He kept my son on Miralax but recommended an OTC med called Prunelax. The instructions were to use the Miralax daily and take 1 Prunelax daily. We could go up to 2 Prunelax daily if needed. Well, we DID NOT need to go up. In fact, we had to decrease the dose to Prunelax 4x a week. Since starting the Prunelax, he uses the restroom everyday without issue. Just an FYI. He does take a magnesium citrate supplement. For severe constipation, our GI recommended keeping OTC liquid magnesium citrate on hand. I hope this helps. I wish you all the best.

I understand your struggle. You may want to consider a GI at Penn Medicine in Philly. Penn is great in general but they also have very knowledgeable doctors. Obviously I haven’t been with everyone who works there, so don’t be afraid to press a bit for tests and new treatments if the doctor you get is a little reluctant or isn’t taking you seriously.

@keeg1010 - thanks for the info. I'm trying the Prunelax tonight. It has the same active ingredient as the Senokot I've been taking, but the Prunelax has a significantly higher concentration of senna. Fingers crossed!

@zachary777 I tried to get at appointment at Penn Medicine. They wouldn't see me without a referral. So, I got a referral and called in June to schedule an appointment. They had no appointments available with any GI doc in 2026! That was as far out as they could check availability in their system. Supposedly I'm a wait list for a callback when they open up the schedule again but I'm not overly optimistic. Seems like you just have to keep calling and calling.

@vincescs
Good luck! It did take a week of 1 a day Prunelax for my son but once it kicked in, holy moly! He was using the restroom approximately 6x a day, which was way too much. I really hope it helps you like it did for my son.

@standinginfaith I'm sorry to hear about your situation. I am new to digestive issues but am currently recovering from diverticulitis which, I will attest, is something you want to avoid. They put me on heavy antibiotics to kill the infection, but I knew it was also killing all the good bugs in my gut. I've been taking about a half-cup of plain Fage greek yogurt mixed with a tablespoon of jam twice daily (breakfast and dessert) and it seems to have dramatically helped my digestive health. I was chronically constipated before all this but now am quite regular, thank you! Give it a try... it is tasty if nothing else...

@keeg1010 have you considered that your son may be reacting to "glyphosate"? Do a little research to find what foods are sprayed with this chemical. I found that eliminating things like regular flour and oats (went to organic) and buying organic produce has helped me tremendously. (Glyphosate is widely used in USA, but banned in Europe, and now RFKjr is trying to wean farmers off it, but getting hard resistance.) Thank you for the Prunelax refer. (I forgot to add: I also discovered that dairy STOPS my digestion. I can have butter with no probs. You might try that also)
