advice : 1st round of pluvicto sept 2. psa rising no longer responding

husband responded well to xtandi and lupron. psa going up now. after latest pet/ct scan more cancer spread in bones. starting 1st pluvicto. 16 men in group in wal creek calif. advice on side effects please. how many treatments has anyone had...sucessful, worth it....any comments much appreciated. have caregvrs 8hrs/day to help.

Be aware that it works really well for 33% of people OK for 33% of people and not at all for 33% of people.

If you have certain genetic issues, either hereditary or somatic, it can affect how well Pluvicto Works. You can ask for a somatic test before doing Pluvicto To find out if you’ve got Genetic changes Due to the cancer.

If you have BRCA2 or ATM It seems to work better. If you have RB1, PTEN or TP53 They are Pluvicto resistant.

The host of the Reluctant brotherhood advanced prostate cancer forum has had Pluvicto and His PSA stayed low for 11 months. He only had two sessions because that took His PSA down to .02. After 11 months, he’s PSA has hit one so he’s going to be doing something, more Pluvicto, pet scan maybe.

I mentioned that so you understand it can work well.

How many bone metastasis does he have? If it did not eliminate all of them, you could have them zapped with SBRT radiation.

His psa was 900 when discovered. The xtandi with monthly fermagon and now lupron brought PSA to .8. Then psa rising since dec 2025, now 140. had scan in feb, now With aug pet/ct scan showing increase in bones and elsewhere will start 1st pluvicto treatment sept 2. was tested to see if this could pass to our adult son, test showed not genetic. Is anyone else currently receiving the pluvicto? Do you know what side effects the above gentleman you mentioned may have had?

Where in his body has the prostate cancer metastasized - Tissue only? Bone only? Tissue & Bone?

per his PET/CT scan aug 11: uptake within osseous metastatic lesions, mediastinal lymph nodes, pulmonary nodules, lymph nodes in the abdomen and pelvis.
Lymph Nodes: Mulitple retroperitoneal, mesenteric and iliac chain lymph nodes, similar compared with 6/6/25.
Bony Structures and soft tissues: DIffuse heterogenous appearance throughout the osseous structures, in keeping with osseous metastasis. Small fat containing left inguinal hernia. Left hip prosthesis.
increasing and new intense uptake throughout the inferior left iliac bone with SUV max of 62.9 (image 350). Increasing degree and extent of uptake in the central right iliac bone with SUV max of 76.3. Increasing uptake throughout the left acetabulum with SUV max of 61.2.

Multifocal intense uptake remains throughout the cervical and thoracolumbar spine, manubrium, sternum, bilateral ribs, bilateral humeri, bilateral scapula. The extremities were also imaged on today's study. A lesion in the mid left femur with SUV max of 4.9. A smaller lesion in the mid right femur with SUV max of 2.5. Multilevel degenerative changes of the spine.
