Met with oncologist today
Well finally 7 months later had my consultation with the oncologist at OSHU, very personable and informative, I have 2 out of 12 biopsy cores that were 3+4 taken from a .8cm lesion, he said there's no hurry on my decision because of the low volume, he recommends 5 treatments of radiation every other day for a 90% success rate, with adt 95%, but probably would settle for 90% without adt, now if the decipher score which I'm waiting for comes back above .6 or so than definitely would do adt, he said with my active life style and my weight being around 140 lbs, might not be that much of a problem I asked can I do the pill form instead of the injections. I still have a Urology surgeon appointment on 9/28, so I will hear him out also and will make my decision soon after.

The answer depends on the severity of your case. It’s really aggressive and it’s come back then you may not want to do it. If you’ve only had surgery and nothing has come back, then it is not shown to be a problem.
The large cribriform Is one reason the doctor may not want to do TRT. As long as he’s OK with that, you can just get testosterone injections and it usually helps. You would want to get your PSA tested every month for a while.

Sounds like your on the right course. If your talking Orgovyx for ADT I think most insurance providers will cover it.

My part D covers it for a year, at tier 5, which put my co pay at $646 per month until I hit the part D max of $2100. It is no cost to me for the rest of the year, but my MO is hell bent on an 18 mo course for me due to 4+4=8 with Crib and IDC. I'll need to change part D providers this fall for next year or my last 6 months of Orgovyx will cost $2000+ a month. The $2100 max out of pocket applies only to drugs your part D covers, and only for the time period listed in their formulary. Obviously a 6 month course would not be an issue if Orgovyx is covered by @ginger38314 part D or his advantage plan. More part D plans cover it than advantage plans, and I'm only assuming he's on Medicare, he doesn't mention his age.

Looked it up because I didn't want to take Luprone too many risks. If
authorized by the oncologist they will approve it 114.00 a month. But if my
decipher comes in low to medium range will omit it for just 5% gain.

$114 a month is great, you'll get 6 months for what I was initially paying for 1 month. Well wishes for a low Decipher, ADT isn't fun, but most of what I have read here and on other forums, Orgovyx seems to have the least side effects.

My oncologist initially suggested Lupron, I told him I would rather go with Orgovyx and explained my reasoning. He has no issue with Orgovyx, but he told me he has patients that forget to take their medicine every day. Lupron has to be injected in the doctor's office or a clinical setting so the doctor is assured you get your medicine on schedule as needed. I doubt your oncologist will refuse your request. After 2 months on Orgovyx I have responded to it extremely well, with very tolerable SEs.

It’s interesting what a doctor will think about taking oral ADT and forgetting about taking it. Sure, the half-life of Orgovyx Is only 25 hours, But once you’ve been on it for a couple of months, your testosterone is down to rock bottom. You forget one pill two pills three pills it won’t matter. Your testosterone is just not gonna rise very quick much less in a few days it’s gonna take months.
My oncologist never said that but only said insurance might not cover it
sometimes they treat patients like they can't fend for themselves if My
score comes in high than I definitely will take it with the 5 sbrt
treatments but if not I'm just going without it. If the oncologist said
well we only will offer you Luprone than I would say adios, and just settle
for nerve sparing surgery.
