Thyroglobulin increased from .6 to 4.2 in two months
Hello!
I had my total thyroidectomy and 35 lymph nodes removed in October 2024, and 121 mci RAI in February 2025 - papillary.
My labs in March had thyroglobulin at .3, June was .6, and last week it came back at 4.2. My Tgab is <.9 (undetectable) and my TSH is suppressed at .38.
Has anyone had experience with this sharp of an incline? I am concerned, and of course these results come in on a Friday afternoon.
Thank you in advance!

I have not had this happen. But the sharp increase in thyroglobulin may (don't panic) indicate some active cells.
Request a neck ultrasound asap. And perhaps a CT SCAN.
take care.
Yes. PTC. CCDC6-RET mutation for me. RAI did not work. I've had 3 surgeries in 3 and a half years. First was thyroidectomy and central neck disection, second a radical right neck disection and a few weeks ago, another central. I had RAI after the first and second surgery. My endocrinologist is scheduling ultrasounds and bloodwork every 3 months. If it returns on the right or central, will most likely do EBRT instead of surgery because of scar tissue/fibrosis making surgery more difficult and neck stiffness. If it goes to the left side, surgery will work best. It's exhausting but I won't let it win. Doctors say this won't be what kills me. Hang in there, things could be worse. Just one day at a time.

1 Reaction@mcro2121, have you had a chance to review the results of your recent test with your oncologist? Did they re-order the test?
Because RAI is not always effective or more than micro cells were left behind....some of us have had chemo & radiation. I did because they could not get all the cancer cells in surgery. I survived the chemoradiation, a year ago age 72. My scans show no active cells. It was tough but many have done it with success. Perhaps it is an option?
