Gleason is highest so core so 4+3.; What percentage is 4? How many cores with cancer? Location (cancer on both sides usually eliminates focal)
How large is the prostate (will be on MRI)? ROs have cutoffs based on size, the lowest I have seen is 80 so it is more severe BPH that gets excluded.
PNI is not a concern; in almost every biopsy.
All treatments can have bad side effects. Worst is ED, incontinence (both more with surgery); proctitis possible with radiation. Best is you are just dry (90%+ of ejaculate is from prostate so hard to avoid if prostate is removed or fried with radiation).

ED is not my biggest side effect concern. Mine is the blinding headaches, constant burning pain in all my muscles, brain fog, and soaking wet sweats. I would gladly trade all these pains for the pleasure of no erections. The ADT is the cause due to low and unrecoverable testosterone that more than half of the men experience. Now numerous studies show that low testosterone and thus ADT's do not help deter cancer but do cause the horrible side effects.
