Positive ExoDx urine test with negative MRI
I am curious if anyone during their treatment had this scenario and what was done about it. I am being followed with basically elevated PSA, not huge but it went up 1 full point in the last 9 months, from 2.1 to 3.2. I'm 64 now.
This prompted a workup with the ExoDx urine test and MRI with and without contrast. MRI was completely clean, nothing, zero, suggesting BPH, but the ExoDx test is off the charts, like 56 or something. My dad passed from prostate CA at 79.
Also on a positive note , I have a low PSA density, 0.05. So currently I am told I should have a PSA redrawn in 6 months and if it goes up a lot then biopsy will probably be done, but not before.
I guess I'm worried despite negative MRI, with the super high ExoDx result, that a biopsy should be done now? Or is 6 month recheck reasonable?

Jeff’s recommendation for the PSE is really good. It will either validate the ExoDx number and you will want to get the biopsy, or giving more weight to the PSE if low then you could consider the continuing PSA, but the biopsy is not that difficult.

My husband had biopsy done with sedation (which is different than general anesthesia) and he had very nice nap during procedure. He had zero side effects after waking up and during the rest of that day - some mild discomfort in the pelvic area and some tiny amount of blood in urine that day.
What is important for you to know is that there are 2 ways to do biopsy - one is through a colon and one is through perineal region and the second one gives better approach to all parts of the gland and also has much less incidences of infection. So, you should have transperineal biopsy if at all possible.

Thank you for reaching out! After reviewing my MP MRI results Pirades 2 and .07 density and a stable age related PSA in the mid to high 4’s, with two other urologists, both advocating for calm and restraint from rushing into biopsy. I’m planning on doing a newer and supposedly more accurate urine test and doing another MRI in November. Which will be a year. I have started on Flowmax, tadafil 5mg, and a short term antibiotic to deal with prostate symptoms like urgency, slow flow as well as supplements to help my prostate. Some love needed there. I’m also doing a Rife protocol specifically for the prostate. I hope that’s helpful.

The PSE test can tell whether or not you have cancer in your system and need a biopsy. It is 94% accurate much more accurate than the other tests.

1 ReactionMy mri was ambiguous so the did the exodos test and mine was 77, was very concerned and had a biopsy which showed 2 of 12 cores positive for 3+4 one 20% involvement the other 30% rated t1c it looks like low volume but will see what my options are on My follow-up on 7/28. If I can treat without prostate removal and no adt I will consider otherwise prostate removal.
