Reclast prep questions
I will be having my first injection next week. I am interested in what would be the best way to prepare. How much water to drink the day before and how much Tylenol to take? Also is Advil ok to take instead of Tylenol?

It is usually easier with the second infusion.
The endocrinologist who ran the clinical trials offered this suggestion which I keep reprint (often).
"A opposed to administering the 5 mg of zoledronic acid in 100 mL of D5W and infusing it over 15 minutes, for the past 29 years I have had the infusion nurse at Stanford dilute it into 500 mL of NS, and to infuse the 600 mL of now diluted BP over 60 minutes. TRUST ME!! By diluting it and administrating it more slowly, it improves the renal (kidney) safety.
The other thing I do is to instruct the infusion nurses to give the patient 650 mg of acetaminophen (Tylenol) during the infusion. Then I tell the patients to take that same dose (two regular strength tablets) with dinner and at bedtime the day of the infusion, with all 3 meals and at bedtime the day after the infusion, and a final (7th) dose with breakfast the 2nd morning after the infusion. This reduces the patient's risk of experiencing a symptomatic APR from >28% to < 1%."

You might ask your doctor to call the infusion clinic and order the infusion in this fashion. Some clinics (in Korea) are giving a dose of steroid prior to the infusion. The rare patients who are hospitalized after the infusion are given IV steroid. You could ask your doctor to prescribe a medrol dose pack, especially since they won't be available to you over the weekend. No one likes to give steroid to patients with osteoporosis, but I understand that the inflammation is harder on your bones than the low dose 5 day pack. That way you could have fun during your sweet grandmother's visit.

2 ReactionsMy provider recommended hydrating well with Gatorade or other electrolyte-providing drinks for a week before and after and taking tylenol before and after, with 1 hour infusion time. I don't know the efficacy of this advice, but it is a bit different from what I had heard elsewhere on this forum.

1 ReactionI just had my 3rd Reclast infusion a few days ago. Hydrate well the day before, the day of and the next day. Also took 2 extra strength Tylenol an hour before the infusion and every 6 to 7 hours after for the next two days.
