Some people say a "new batch of inflammation" happens every night. That might be true but the timing coincides with the approximate time when an a.m. dose of Prednisone wears off in the middle of the night because of its half life.

Some people metabolize Prednisone slower than other people. The lucky people who metabolize Prednisone slower might be able to get a full 24 hours of pain relief from a single morning dose of Prednisone but some people don't get 24 hours of pain relief. Prednisone has a rapid onset and short half-life, clearing your system in roughly 16 to 22 hours. Fortunately the rapid onset of prednisone is a quick fix for the problem of increasing inflammation levels whenever it happens.

As you probably have learned the inflammation comes back as soon as prednisone is eliminated from the body. The circadian rhythm is a characteristic of cortisol from the adrenals in the body and not a characteristic of oral Prednisone.

@dadcue Super helpful. At day 25, I can report that my early morning 15 mg of prednisone does indeed last me a full 24 hours. The first couple of weeks I had some AM shoulder and hip stiffness, which had me reaching for the Vitamin I, but that is currently gone. My stability has improved dramatically; somehow my ankles have loosened up (I wasn't aware they were restricted).

I've seen a massage therapist three times to rehab the injured left shoulder (and give an assist to the underused other shoulder). The MT and home PT that had done literally nothing to help for a year has now got my shoulders pain free with ever-increasing range of motion and strength.

I'm losing weight with ease (which is key for me). The absence of pain and the prednisone boost make it so easy to stay on plan.

I'm going to enjoy these last five-ish days of 15 mg.

After that...I've got my plan ready to go. Don't know what I would have done without the wealth of experience presented on this board.
