Docetaxel vs Pluvicto

Hi,
My husband's PSA is up to 400.
He's had one chemo of docetaxel, but only 1/3 dose because he's still recovering from spinal tumor at T4 which was removed mostly by surgery and radiation in November 2025.
Shouldn't the docetaxel work right away? It's a low dose, but still his PSA went up from 365 to 400 in 3 weeks...
The Dr. is going to give him 2 more treatments with docetaxel over the next 6 weeks, and has mentioned Pluvicto..
Does Pluvicto work? Is it as good as docetaxel? Does docetaxel always work for everyone? This is all so scarey...He was doing so well...Last October cycling 150K a week, mowing the lawn...
Any advice...? Obviously the cancer is spreading if his PSA is going up...Shouldn't they start Pluvicto now???

What protocol do they use where you’re getting tredfme? Do they require chemo first to qualify for Pluvicto?

@brianjarvis "Falling through the Cracks". I talked to my health insurance today. Software thought I was out-of-network so $1000 copay for CT Scan. But it's really $375 copay (up from $300 last year). Besides I was approved for Hospital Financial Aid 100% . So I can get the PSMA PET Scan for deciding the next treatment. Docetaxel is more side effects. I have the BRCA2 mutation better for Pluvicto. Both are infusions. I hope I can find an oral pill again. Stopped Lynparza, became ineffective, making me sick, much better now. Same with Xtandi. Degarelix infusion for hormone testosterone almost killed me. Lupron infusions worked well except for a really bad injection & later castrate resistant (cancer doesn't care about testosterone). Thanks everyone. I hope I'm back on track financially & administratively.

@brianjarvis first part. My impression is a CT Scan will lead to Docetaxel. A PSMA PET Scan could lead to Docetaxel or Pluvicto. It's an advantage plan. Turns out their software thought I was out-of-network & quoting huge copays in error. The Billing Crack.
