I hesitate to make a recommendation but you seem desperate. Have you seen @sailnsun's report on his revision:

My magician is not cheap and he doesn't take insurance but he is an incredible surgeon. He replaced my left hip four years ago and my right knee six months ago and I had zero post surgery pain each time. He likes to use the Smith & Nephew Journey II implant which is made of oxidized zirconium and is allegedly biocompatible.

Thank you for the recommendation. We are traveling to DC in July to meet with the surgeon in person. We've already had a phone consult and he's willing to do the revision surgery, unlike the past 7 surgeons we've had consults with. We might just travel to AZ to meet with your surgeon.
