How long for Decipher Results to come back

Hello. I was recently diagnosed at Mayo Clinic Phoenix with multifocal, bilateral 3+4 favorable intermediate-risk prostate cancer, with 2 cores showing 95% involvement. The 95% involvement worries me the most.

At my post-biopsy consult, I agreed with the urologist that RALP was the best treatment option and he recommended a surgeon. In the last minute of the consult, I asked if a Decipher test was warranted (as suggested by ChatGPT). The urologist seemed hesitant and maybe peeved but agreed to order the test. However, my progress towards a surgery referral has stalled while I wait for the Decipher results, and the urologist hasn't responded to my referral request on the Mayo app. I am over 2-weeks post biopsy.

I am anxious that my "favorable" risk assessment could change, or that the cancer could progress while I wait. Am I overreacting?

Probably. Depends on two factors you did not put in your post: What was the percentage of type 4 in your biopsy cores. Much more important than the 95%. If it was 45% it could technically progress, but that could happen if you sent the slides in for a second opinion and it came back as 50% type 4. The other factor is how fast you PSA was rising. If it is rapidly increasing than you could have very aggressive disease that was caught at a very early point. This is not likely. In general prostate cancer is slow growing, especially at the early stages. You might want to ask some of the former patients on this forum for surgeon recommendations since that will be the biggest factor in how well things go for you.

My Decipher test took several weeks to get sent off for some unknown reason! However, once it was sent off took about a week to get results. Did they do a PSMA Pet scan, that might be good?? Also, they did a genetic test on me. Ended up having RALP on 4/30/26, things are a bit difficult, but hoping for things to get better.

It is good you ask questions and gather information. Lots of good resources on this site.

Thank you for the response. I am still trying to figure this all out and understand the issues and terminology.

Here are the doctors notes on the biopsy

"Recent prostate biopsy showed adenocarcinoma in 5 cores with predominately Gleason 3+3=6 and Gleason 3+4=7. Recent prostate biopsy showed adenocarcinoma in 5 cores, with predominately Gleason 3+3=6 and Gleason 3+4=7. Targeted right posterior medial cores had up to 95% involvement and additional targeted cores up to ~55% involvement; one core had a Gleason pattern 4 component up to ~30%."

But the biopsy did say:

"Recommended block for additional ancillary testing: Block I2."

Which is why I thought the Decipher test might be useful but what do I know.

With a 3+4 Gleason, you are unlikely to have any problem with cancer spread for a while. Yes, it’s high involvement, but it’s really not a big deal, It’s very slow growth with a 3+4.. I had surgery 16 years ago with a 3+4 from the biopsy and a 4+3 from the surgery prostate biopsy. I’ve had four reoccurrences and I’m 78 now and quite active.

I’m still here and I have BRCA2, which makes it much more aggressive. I’ve been undetectable for the last 30 months because the drugs are so good.

Decipher results usually take 2 to 3 weeks.

With a 3+4 radiation is also a very good option. You should speak to a radiation oncologist. Make sure you’ve look at both options.

Prostate cancer is pretty much a chronic disease rather than a fatal disease. Almost 90% of patients die from something else.

Don’t let the anxiety get to you, you have a lot of time to make a decision.

Thank Jeff. It is comforting to hear from others that have gone down the road before. And the drugs and therapies are getting better ever year.

Next Monday Ancan has their low/intermediate risk group meeting. This meeting occurs twice per month. Join the meeting about 10 minutes early and they will ask if you are new. They will go over your info and can recommend surgeons in your area, Radiation Oncologists in your area, recommendations based on your MRI and biopsy, etc.

Excellent small group setting to ask questions and get feedback.

Last meeting, they indicated this meeting would occur even though it is Memorial Day.

I am a 3+4 with two cores over 90% involvement. Yesterday, surgeon and I reviewed April repeat Biopsy. I do not plan to have surgery before September. I want to enjoy the summer with family. Surgeon agreed this is a good plan.

In the meantime, I will have results of Prostox test in the next two weeks.

Surgeon’s staff will be working on getting genetic testing completed for me and sending biopsy slides to Dr Jonathan Epstein for his 2nd opinion of pathology reading.

Retzius-sparing surgery is one of the important advances over the past several years.
