Not Good News after prostate biospy when MRI didn't look too bad
Last month I had PSA of 5.23 when a few months earlier it was 3.2. Then they scheduled me for MRI of prostate. Did another PSA and it was down to 4.16, but still wanted the MRI. Report is below, doesn't look good PI-RADS 5. At one point they say in report Lesions (PI-RADS 3 or higher). If I understand it, it hasn't spread. Wish I could get a plan with doctor!

Transition zone: No index lesion. Stromal and glandular BPH nodules.

Prostatic capsule: Intact.
Neurovascular bundles: Not involved.
Seminal vesicles: Not involved.
Lymph nodes: No lymphadenopathy.
Bones: No acute osseous abnormality.
Other findings: Small fat-containing right inguinal hernia.
IMPRESSION:
1. The prostate gland measures 5.7 x 5.0 x 4.9 cm with volume of 68.75 cc. PSA density is 0.06 NG/mL/CC. 2. Lesion # 1: PI-
RADS 5 lesion in the left posterior peripheral zone extending from the base to the apex measures 5.83 cc. No frank extracapsular
extension. 3. No pelvic lymphadenopathy.
PI-RADS Category 5: Very high (clinically significant prostate cancer is highly likely to be present)
Really doesn't look to bad, one spot that hasn't spread!
Then Bad Update 2/10/2026
Well got biopsy yesterday and results today, doctor hasn't called, just sent biopsy results to MyChart.
The MRI showed only one Lesion like shown above. Had biopsy done yesterday, they did 3 from the Lesion and 6 from each side of prostate. I wondered why they did more biopsy that were outside the lesion, but didn't ask. Got report today- not good. The lesion look better than areas where MRI saw nothing. They took 15 samples total.

Another thread I posted in a person said "You have a Gleason 4+3 7 BUT you have large cribriform and doctors a UCSF say that puts a 5 in your Gleason score." I believe he picked this up from the biopsy report. I don't know what a cribriform even is, it's not mention in report. From googling around it can only be determined by sieve-like or "Swiss cheese" appearance under a microscope and I don't see that in report? But this is all new to me. Doctors haven't talked to me yet, who knows when they will call or make appointment, took long time to get MRI and even longer to get the biopsy done. Sure were fast getting results, they said 7 - 10 days and they gave them to me the next day. Kind of wish they didn't give me results prior to talking with me.
My first thought is just get the thing cut out, not sure how that is done, as seems they got to leave something in there for urine to flow threw. So they couldn't take 100 percent of prostate out. Then I read about nerve sparing or not and not sure what that means. No doctors have discussed this with me yet. Seems if they take it out there shouldn't be any prostate cancer left? But then I read where people get it out and still have a PSA level, so like I said earlier, they must leave some in there, even when they call it total. Had to drive 150 miles to get MRI and biopsy They could have done that in Topeka, but KUMC is ranked as number 50 in top of prostate treatment so I went there Topeka doesn't have a Proton device, that would be back up to KUMC 150 miles RT. One of those radiations therapy is only a few days, not 30 some days. They do have SBRT radiation in Topeka, but I know of someone who had SBRT or maybe it was IMRT and it screwed up several other organs around the prostate, like bladder, kidneys and intestines.
Then some tell me I am lucky to have them all in grade group 2 or 3. But seems like I had a lot of them (12 of the 15) . So I would guess if they did 25 biopsy I could have had more grade group 2 or 3.
All confusing and stressful, other that this I am 78 years old healthy as a horse- no other issues and very active. Loss of what to do and all the different radiation types, that why just getting the pesky thing cut out of there, but seems they still leave some in.

Well unbelievably the surgeon set up an appointment to tell me about surgery and let me ask questions and he will check me out. I sure do notice that is I move around like they say, they tell me to walk and stay active. I do it !! When I am activity my bag is bloody red, but I do it anyway. I dump it before bed and when I get up it is all yellow- no red. This morning it was so full, the measuring cup wouldn't hold it all, it only holds 32 OZ. I had an overnight drain of 42 oz and all clear. But not clear now as moving around even though it hurts. Bought me a new bag today that I attache to lower leg. Well have to empty more often, but can walk around without big bag flopping around on outside. Will have to use big bag at night, but pretty easy to change. Pain to clean, don't see why to clean. If I didn't change bag they say it don't need cleaned.
BTW Tomorrow I get to ask questions to surgeon. I have a few. Any of you guys got a question you think I should get an answer from the surgeon???? Well I am heading out to do a 2 block walk. No more 3 mile walks for awhile. The 2 blocks are a little tiring , but not bad. Will cause more red, but already have that from my 648 steps in house.

It seems like a lot of blood in your bag still to question him about. Since it occurs when you are active but at night your urine returns to yellow, they might have you rest with no activity for a day or two to give time for what is bleeding to permanently clot. If you want to keep your big bag in the house, it is easy to carry it around in the house in mop bucket. Just drop your bag in it. Another is a firmer shopping bag .

2 Reactions@diverjer You do not want to sue. Lawyers cost a lot of money and if you recover you really do not have the damages required to have one take the case on spec. Now if they have to amputate than you will not have any trouble suing and getting a good settlement.

Yes, when I went for my short 2 block walk, blood in the line. Sit around house now for about and line has turned normal color.
I like the leg bag during day, even though I have to watch it more as it get full quicker.
Carrying the big bag wasn't terrible, had clips on it that I put on leg. Just didn't care for being out in public with bag flopping around. Just need bag lower than bladder. With leg bag, you don't even know I have anything.

As do I, the spot below where the JP drain was has turned black and blue. The bandage over the drain hold has saturated with blood, but dried and tape coming loose. They said leave alone 3 days. That would be tomorrow, but I am hoping doctor staff will put new dressing on. Everyone from LPN, RN, charge nurse and out daughter said that was not right. I could have come home with it and measured and dumped, but said no. On my ankle replacement I had one, but much bigger than JP, they have another name for it, but works same principle. Had no problem emptying it. They even sent me home with a big ball that works kind of opposite of JP. They pumped it full of some kind of nerve pain killer. Really long tube weny in leg and next to a major nerve. When the ball was down to nothing, I just pulled it out, had a really long tube, 8 inches or so. Didn't hurt at all.

I had that same ball of painkiller when I had my knees replaced. Lasted for three days. Those knee surgeries are really painful so you really do need that kind of pain relief. Also had to pull it out when done no big deal.
I sure hope the problem you have with rhe leaking inside from the lymph node removal gets resolved soon. They sure did seem to do the wrong thing. There was another guy in the forum, complaining about having that drain for so long, They could have left it in.

1 ReactionIt's getting bad, too much still leaking inside. I already said how balls and penis are horrible. Now just today below where J P was is black and blue clear across from one hip bone to the other. Now it's about 3 inches up and down. You can tell it coming from JP sight as much darker there and bandage over the JP hole is socked, they put a big bunch of gauze and tape that is all dried blood. It all hurts, not sure which worst balls or incision points there are five of them plus the JP incision point. I believe the testicles are the worst I put ice on them and I put ice on the incision. It's really horrible looking I did take pictures in case I ever decide to make complaints, but there's just too nasty the post on this site, really gross..

Maybe they had no choice because of healing/support considerations…I’m not a surgeon so I couldn’t stay.
But the bleeding thing is concerning so stay on them if it doesn’t subside soon! Best,

Don’t hesitate to go to the ER even for your own self assurance. It does not sound right. If more skin is continuing to black, i would not wait.
