Two years post proctectomy with pelvic floor pain

Hello,

I had radical proctectomy via da Vinci robot two years ago. At first, I was fully incontinent, using 10 pads per day for about three months. I started pelvic floor physical therapy, and my pad use went down to about 2-3 per day at the 5-month post op date. But, during my physical therapy I developed very tight pelvic floor muscles which clamped down on my pudendal nerve causing pain from the tip of the penis to the rectum. My urologist suggested a nerve block, which helped the pain for about a day. The urologist then suggested a bot@x injection to relieve the nerve pain about 5 months ago. The pain was reduced by about 25%, but fully incontinent again after the injection.

I have changed urologists to a larger learning hospital in Wisconsin. The new urologist is suggesting the use of bot@x for pelvic pain was not a smart move (I realize that now), since I still had some incontinence at the time of the injection.

I'm planning to have either an AUS, or the implant of ProAct device.
I will have urodynamics testing soon. Will the Urodynamics results point me in the best direction? My new urologist is concerned with my hypertonic muscles and nerve pain in the pelvic region to perform the AUS. He is telling me that I should expect to be about 80% dry with the ProAct?? Any suggestions and input would be great and appreciated.

You had surgery and one reason might be that you knew you could have Radiation if it came back. If you have radiation, then the ProAct will not work. When you have radiation, it hardens the urethra, ProACT Places balloons on either side of your urethra and presses on it to shut it down. When it hardens, it is unable to work properly.

I not only saw a presentation by urologist and the last year, who put it in a lot of of his patience and then found this out and had to redo them. They also talked About it, not being usable In that situation, today, at the UCSF conference I went to. There are a couple of new devices that will replace the current AUS and maybe they will work for you.
