Should I have an Aquablation?
I'm 67 years old and was diagnosed with Prostate Cancer this past fall. A routine PSA test showed that my PSA jumped from 2.1 up to 5.7. So my doctor sent me for an MRI which showed I had a small lesion (2mm) that the Radiologist suspected was cancer and I then had a biopsy which proved it. I have a very slow growing cancer and doctors just want to watch it right now. The MRI also showed that my prostate is enlarged but nothing crazy at 79.5 cc. However the MRI also showed that my prostate is growing up into my bladder 26.4 mm. They called it an Intravesical Prostatic Protrusion and the doctors were concerned about it and recommend that I have an Aquablation to remove the protrusion and to reduce the overall size of the prostate. I thought about having my prostate removed but there are too many adverse side effects doing that. The doctors tell me with such a large protrusion that I'm prone to bladder infections and other problems. I'm not sure what I should do, the aquablation seems so ......

Another one to look at is proatatic artery embolization/ PAE , which is a very easy procedure. I couldn't tell you what is best in your situation, but these procedures work.
I asked about using PAE and the urologist had nothing bad to say about the procedure, except that it probably wasn't the best procedure to address my issues.
Is there some reason they picked anquablation over other focal therapy techniques? I have absolutely no idea what would be the best treatment, But it definitely sounds like you need to do something.
At the Wednesday Mayo Clinic online prostate cancer meeting the doctor talked about radiation treatments. He did say that working with a prostate that was over 75 cc was a problem. Doctors prefer not to radiate when it gets that big. Some will do it.
I just bring this up because you have a problematic prostate and maybe that aquablation is a solution
I would consider Tulsa Pro. It will take care of the enlarged prostate (it is sometimes used only for that) and the cancer.

@jcf58 Am I reading this correctly? It seems to good to be true! I can have my small cancerous lesion and enlarged prostate taken care in one MRI guided procedure??? No more waiting and watching the my tiny cancerous lesion to grow? This procedure sounds like the answer my prayers!
I don’t know if it can handle your protrusion but it definitely can handle BPH and a lesion. There are a few guys on here who have had very large prostates and cancer and treated both with one Tulsa procedure. I had a 45cc prostate and had 30% ablated. Definitely improved urine stream! Good luck.
@jcf58 I also have Gleason 6 prostate cancer. I aquablation for BPH 9 months ago. I'm glad I had the proceedure because I can now urinate very well. I have been interested in the Tulsa Pro proceedure though. I am on active surveillance and doing well. If it bothers you to have the cancer, you may consider the Tulsa PRO proceedure and take care of everything at once if that's possible. I am not a doctor though. Do your research BPH can cause problems if not treated.

How did the doctors evaluate that you had a slow growing cancer? Did you have a decipher test? My brother had aggressive Prostate Cancer with a 6.5 PSA and I did not have aggressive cancer with a 10.2 PSA.
After my PSA jumped from 2.04 to 5.74 my doctor sent me for an MRI. The MRI found a small suspicious 2mm sized lesion, It also showed that I had an enlarged prostate (79.5 cc) with a 26.4 mm intravesical prostatic protrusion growing up into my bladder. So the doctor did a 16 sample Biopsy of my Prostate that showed that I had adenocarcinoma with a Gleason score (3+3=6).
Last week they tried todo a bladder endoscopy, but aborted the procedure because they could not get the scope up my ureter. So now I know why it's often difficult to urinate. They also retested my PSA and it is back down to 2.16.
