Exemestane side effects

Posted by brightlight66 @brightlight66, Sep 16, 2025

I've been on Exemestane for two weeks and have abdominal discomfort...gas, nausea, rumbling, diarrhea, etc.
Thinking of just getting off it as I can't imagine feeling this way for 5 years. Any other experiences out there? I was diagnosed with invasive lobular and already had surgery, chemo and radiation.
Thanks!

Loratidine is a second generation antihistamine. Generally considered safe with minimal side effects up to a year. After that usually recommended only under supervision of a medical professional which you are doing. I’ll seeif I can find the original article I was reading and I’ll let you know. Long term use (2+ years) have been shown to cause liver problems, hypertension, tachycardia. Incidence is LOW which is why one should be monitored.

Hi, I also have ILC had a double lumpectomy, chemo 4 rounds, hypofractionated radiation for 5 days. Finished treatment June 2026. Was on Letrizol but joint muscle bone pain too intense. Moved to exemestane 2/20/26. By early to mid March had gas, soft stools that turned to liquid diarrhea and lots of stomach gargling with on and off nausea. I took Imodium this has helped see my dr tomorrow. I know these are side effects of this medication. But also have joint & muscle pain on this mostly in shoulders, shoulder blades and lower back. Others have similar experiences? Thank you!

@warrior1usa sorry it is so difficult. I have been on exemestane for 3 years with no diarrhea but, lots of ongoing joint and muscle pain, mornings are the worst. As I am stage 4, I just hope that it is keeping me alive.

Hi! @warrior1usa, @mspass:

I am so sorry that you've been suffering through these painful issues:( I only wish that we may provide some experience to mitigate these problems somehow...

Even though I've never been on Exemestane medication, I, too, have been experiencing muscle, joints pains all over my body, and neck & spinal issues through the years ever since the surgery to get rid of the IDC tumor (12/2023) and start my ongoing BC treatments (01/2024) to prevent its recurrence or metastasis. Fortunately I've been mitigating these various aches and pains, and degeneration issues with physical therapy sessions and they help me tremendously. Perhaps you may consider asking your care team to refer you to a reputable PT organization - preferably a rehab center for cancer patients. Simply wish to reduce your pains, my friends.

Wishing you all a better journey ahead with hope, peace and love!

Thinking of you! I am going back to Letrizol and they have ordered me PT for my surgery breast. I still have an awful lot of tenderness and discomfort. I work out seven days a week so I’m sure exercise is part of that but they said so many nerves were cut and disrupted that PT may help. I understand you can ask for PT for a whole body joint and muscle pain so maybe you could look into that?

xoxo here for you!
