UTI found prior to biopsy
My husband following an elected PSA and subsequent MRI showing two lesions and probability for cancer has been scheduled for a transperineal biopsy seven days away. The nurse from his primary care physician called today to let him know his urine sample (sterile catch) came back positive and the doctor prescribed Amoxicillin 3x per day for 7 days and to drink plenty of water. Wondering regarding the safety of doing the biopsy under this condition? Wondering how they know if the Amoxicillin was effective against what ever pathogen he has before doing the biopsy? Perhaps someone else had a positive UTI at the pre surgical testing for a biopsy clearance or is knowledgeable on and could shed some light on this issue? Thank you!

Should not be a problem. Transperineal biopsy is very precise and should not come anywhere near the bladder.

2 ReactionsI don't really know, but I tend to agree with @heavyphil that it seems like it shouldn't be a problem. But you might save yourself some grief and contact the doctors office who's doing the biopsy and inform them. I suspect they have a protocol for someone showing up under care they didn't previously know about that might trigger some action on their part. It'd be a drag to get there and then have them say they can't do the biopsy until he's off the antibiotic. All that said, since they're responding to lesions on an mri and not just a psa rise, it seem to me the biopsy is still needed. Best wishes.

2 Reactions@retireditguy Thank you. I called his urologist’s office and waiting for a response. The procedure is done at a different location and doctor there or staff is not reachable for patients. Hoping someone else in the past had UTI prior to biopsy and could learn from their experience. What others think is also helpful when your new to all this

1 ReactionI have often used a walk-in lab for simple procedures. Any Lab Test Now is the one I have used and they do a good job. Maybe go to a walk-in lab and have a urinalysis, if all is clear then that will ease your mind.
@jeff1963 Thank you for your reply. I looked them up and none are located where we live. Good idea but seems that you need a prescription for the lab here. When the doctor’s office calls back I will inquire.

1 ReactionI've had both transperineal and transrectal biopsy. For the transrectal I was given a dose of antibiotics to take as "insurance" against any infection. About 6 months ago at my annual PSA test (I am on active surveillance for Gleason 6) my test number jumped from under 1 to 8.4. My urologist immediately suspected prostatitis as I had a UTI 6 months prior to the test and I guess that can cause prostatitis. I was given antibiotics and retested 2 months later at which point my PSA came in at a more normal 1.2 I would guess that the UTI would not impact the biopsy but you are wise to let the doctor know that will be doing it just in case there is some reason they want that cleared up first. I will say a prayer all goes well for your husband.

UPDATE: Our primary care physician doing the pre surgical testing would not give clearance for the biopsy. Reason is risk of sepsis greater than a delay in biopsy. Husband was instructed to go to a lab Tuesday for sterile catch urine analysis. Biopsy scheduled for Tuesday with the specialist urologist who exclusively does these biopsies will need to be rescheduled.
