Lumpectomy with mixed ductal and lobular features and hyperplasia?

Posted by lkat95 @lkat95, Jan 11 10:03am

I was diagnosed with stage one invasive carcinoma with mixed ductal and lobular features in November. Since that diagnosis, an additional area in the same breast was diagnosed with atypical ductal hyperplasia. When I am talking to doctors, a lumpectomy seems reasonable and I leave feeling confident with my decision, BUT I spend the next nights, days and weeks agonizing over that decision. I can’t find much from people who have had a lumpectomy on this forum. Most seem to go with a mastectomy. Is there anyone willing to share about their lumpectomy experience?

I had RB lumpectomy, followed by 15 radiation treatments and anastrozole in fall 2024. Lobular, 22 mm, highly ER+, Herc-, BRCA negative, low Oncotype score. July 2025 mammo led to LB lumpectomy for 8 mm tumor, 15 radiation treatments, and now exemestane for 5 years. Very good prognosis for both, but cancer is sneaky.

Lumpectomy is the easiest surgery I've had in my life, and I've had a few! I took ibuprofen and tylenol for post surgery pain, and was back to work in two days. Neither surgeon (I changed hospitals) recommended mastectomy. So much depends on the biology of the tumor, your age, your preferences. For my type lumpectomy, radiation and AI has same outcomes as mastectomy, and I like my breasts!

@lkat95, what did you and your cancer team decide? How are you doing?

I am 64 years old, a bit overweight and have/had large breasts. I was diagnosed with both lobular and ductal early stage(1a-2b with no lymph node involvement) in my right breast, and had oncoplastic surgery with a left breast reduction at the same time on 2/19/26. I am fortunate to live in an area of a well regarded cancer and research hospital, and my plastic surgeon is very well regarded.
When I heard there were two tumors, I thought I was going to need a full mastectomy, but my surgeon explained to me that since they were near each other, we could do the lumpectomy with radiation and have as good of an outcome 10 years out as a full mastectomy would be.
So far, I am happy with the results, and still have fairly good size breasts that are symmetrical. I have fairly good sensation in both breasts and nipples. My swelling has begun to dissipate. My breasts sit “high and wide” but I understand that they will “settle” over the next couple of months.
A lot of the recent research shows scientifically that lumpectomy with radiation has as good of recurrence outcomes as full mastectomy.
I’m happy that this was the option I chose.
