Do you have urinary incontinence after prostatectomy & radiation?

Posted by hmctgraybill51 @hmctgraybill51, Apr 16, 2022

Hello, I underwent a prostatectomy in Sep 2017. Gleason score of 9 with a positive margin. 8 months later began radiation tx with rising PSA. As of this date my PSA's have been undetectable however, I continue to have moderate to severe incontinence. I have seen two urologists with one suggestion the male sling and the other the artificial urinary sphincter for relief. I'm looking for any men with experience in either of these procedures and your comments. Thank You

Sam60, thanks so much for sharing your details with AMS. The surprises must be terrible both mentally and emotionally. Hope you can find a workaround to prevent such things.
There must be some real variation in the way these things operate. After I find and skin back my penis it takes a fairly hard squeeze to activate AMS. But then it’s great (as long as my aim is decent). And maybe one more good squeeze and I”m empty. My only worry is that the arthritis in my thumb(s) progresses. Even if I have to use a pliers I”m not going to give up.
I hope. You can find some adjustment to the AMS that can hep.

Tomf, I encourage you to keep bugging your friends about PC and to keep checking their PSA. I’ve badgered my two sons, both in their 40’s, till they got a baseline PSA. And same for friends. What surprised me is so many of them also had PC but never mentioned it.

We all gotta spread the word. We could save a life or a life-style.

Good luck to both of you and all our PC brothers. We need it with God’s help.

Same here, after radiation, AND the Eligard every 6 months. Not really incontinence, just urinating all the time.
Drink a 20 oz bottle of water, pee every 15 mins for 3 hours. Annoying and disruptive. Anyone else have this?

Please read my previous post. I had this problem and was treated with SANS (Stoller Afferent Nerve Stimulation). Dr. Stoller is vice chair of urology at UCSF and was my doctor. This procedure is non invasive and is widely available. Sort of like a targeted, enhanced acupuncture. It really worked for me.

I just completed 45 ,nine weeks of radiation treatments. Now dealing with total incontinent issues. I progressively loss controls over the last few weeks of treatment. Any real hope of regaining some controls as recovery progresses. Do any of the exercises I am reading about help? Any suggestions would be appreciated.

My husband had 2 rounds of Bulkamed injections after his prostate removal. It’s kind of like Botox. He was very satisfied with it. He most likely will have one more after he finishes radiation.

我的手术是根治,2023年元月7日手术,至今尿失禁。有问过医生吊带和安装人工扩约肌均有问题,所以我不准备这些。现在在做提肛,效果不出院时好些。估计这辈子就是尿袋相伴了

Hello Berg55,
Colleen gave you some good sources, I’m sure. Check them out.
FWIW I can share my incontinence story for your info.
I had very similar RP January 2022. Similar good recovery. Did Kegels before and after RP. But I guess I lost too much of the sphincter likely due to a large prostate, 70cc.
Then, total incontinence. Of course this is the WORST possible urinary outcome. I was really bummed about the whole mess - - literal mess!
For about 15 months I struggled with Kegels. Never saw any improvement.
But like all of us in some ways or others, we cope with the situation and try to find the best response we can.
So at 75 years old I became an infant again. Yeah, it was a spirit crusher but nobody knew unless I told them. So how to cope???
I used Prevail brand briefs though I only could find them on Amazon; no big deal as their price was typical to others; they were just handy and not as bulky. Also, I’d get “Depends Guards for Men, 52 count”. (Luckily I found a much cheaper, and I felt better, store brand at out local chain, Meijers).
At the 15 month mark I got an AMS 800 artificial sphincter. And by a great surgeon too. Though I had to wait about 5 weeks for all the healing to be completed beyond any doubt. Then I had it activated. Right off I went from 7 to 8 pads + one brief per day to one pad and only the brief to hold it in place. In a week or so, I was comfortable that the new rig was working, I got smaller pads and regular jockey shorts. Less bulk!!! More comfort!!! And able to stand up to a commode (though I had to put my pants down since the AMS is a two handed operation) like a man!!!!!!!
What a spirit booster that was. Now I only wear a thin pad as another “just in case” though I’ll be tryin out NO supplemental pad right after the Holiday travel is over. And, yeah, I’m taking it slowly. I just don’’t want to relive that incontinence in any way.
Add that to my lifted spirit and now life is GOOD. No more changing pads every 2 hours! And I can go as much as 5-ish hours if I need to; longer means I’m not drinking enough.
So should the worst happen to you I am confident that you will cope and survive; and hopefully survive well.
So Kegal away and plan for the best but know what to do if it’s the worst.
Be strong, be positive, be creative and with the Good Lord’s help you’ll come out the other side with a great life to live.
All the best!!

Thank you for your detailed explanation of your experiences. I have just had my RP on 1/12/26. I am so fearful that I will have a similar experience like yourself. I am 57yoa. My prostate was very large at 95cc. Surgery took 5.5 hrs vs the estimated 3hrs. I had to have catheter for 2 weeks and after getting it removed I have zero bladder control. If I move, I am peeing. I get some relief laying down but when I sit up, peeing. Then stand up more pee. I have spoken to Dr's nurse and all I get for a response is "it's way early, healing takes 3-6 months." Well I am extremely worried that continence will never return. I too did kegals before surgery. I am told to wait 6 weeks before continuing kegel physical therapy. This is why I am here. I have searched far and wide for some direction and answers. Oh I forgot to mention my surgery was supposed to have successfully spared the nerve bundles. Has anyone transitioned from total incontinence to continence?

@mgarrett9ball I am 7 weeks post-op and my situation is similar to yours, maybe I am a little better and maybe I can see a little progress over time, but still mostly incontinent when walking for more than a couple of minutes.

I am remaining hopeful that it will get better, doing kegels, working on getting approval to start PT/pelvic floor training, surgeon wants me to start on drugs to treat possible UTI and drug to stop bladder spasms (have not started either drug yet).

Have finally gotten into a better routine (pads, pull ups, changing throughout the day, etc) to keep myself dry and skin healthy.

It's easy for me to get discouraged, but keep thinking of the 3 month milestone, hoping for improvement by then. Also count my blessings that (as far as I know) surgery has made me cancer free, and small setbacks are worth the price.

Good luck! Hope this gets better for you.

@mgarrett9ball
They do say during some of the conferences about prostate cancer that continent can come back within a year for most people. You are very early in your post treatment.

There are many solutions if your incontinent is not Improving significantly over the next few months. This is not a problem that will last your lifetime, If it continues for a long time, you can take advantage of one of Treatment options available. Hopefully your case will be like most other people, and you will get continent back.

@jeff1963 thank you for your encouragement. I tried condom catheter today for the first time and I am really impressed by how it has affect me physically and emotionally. I am learning with research that it's not a 24/7 fix but to go without pads for even a day is well worth it. I purchased on Amazon and can't say if one is better than another. I think getting the right size is key.
