Swollen stoma: Do I need to worry about this?

Posted by hheter @hheter, Dec 12, 2025

I have had my ileostomy since June. Hardly any problems at all. Tried various bags and supplies that I did have trouble with- some leakage due to quality.

Anyway, my stoma today is super swollen and seems to be protruding more than ever. The wrinkles that were there before are now just a solid bulge.

Do I need to worry about this? Will it go back down? Do I need to see someone? It doesn’t really hurt. There’s some pressure but it’s uncomfortable in the bag.. help!

I would think that unless it continues on and on, it's not a problem. Could you cut the hole in your flange/barrier/wafer larger until the swelling abates? Your stoma is very new and they do change. I originally had a nearly retracted stoma but now it's more out there...sometimes...and sometimes not.

I have had an Ileostomy for 7 years now and occasionally my stoma protracts outward and is swelled up. I have no idea what causes the spasm but I just wait and in 3 or 4 days it goes back to normal. I used to have a lot of problems with bag leakage but then discovered if I cut the hole bigger in the base plate the leakage past the adhesive piece was not a problem any more. Hope this helps.

I just recently had an unexpected Colostomy much to my dismay. My stomach was large as well. When I had my first follow up with my Surgeon he (unfortunately I can’t remember what he called it) just pushed it back in. He said this sort of thing is fairly normal.
My biggest problem right now is infection and so much antibodics I can’t eat and getting weak.

@fckaiser
Please explain more about what eliminated the leaks. I'm not sure what you meant by "base plate" or "adhesive piece". Perhaps pictures would make it clear.
Did you cut the hole around the stoma bigger? That would expose more skin. Generally not good.
Are you using a two-part system, with a "base plate" and pouch?

@pidge32 this was helpful!
I notice that it’s small when I wake up and gradually gets bigger throughout the day. So today I put leggings on to hold it in. Much better so far. You gave me th thought that it can retract with pushing it back as the doctor did so I think this will help hold it in and keep the size smaller. Instead of pushing out all day more and more.

@piki thank you! I’m sure you are right it will go up and down and change. And if no pain I’m ok.

Thank you everyone for your input.
My doctor told me to wear a binder if I’m going to be having an active day.

My stoma also varies in size at different times. I was told by a stoma ,nurse when it swells up it is trying to push stool out. The best thing is to keep your stools flowing It also helps when me when its not swollen to know I can take a nice hot bath without having any incidents

Is it important to keep your stomach covered when taking a shower. I have had different answers from different doctors!
