What happens after Tymlos?

Posted by alknapp @alknapp, Feb 21, 2024

I’m going to be finishing the 18 month course of Tymlos in April/May of this year. Endo is saying that I should follow that with Prolia (within weeks) and then Reclast.

Has anyone else followed that treatment plan or have any comments about it?

Thank you.

Just want to add that I am going to do a 20% dose of Reclast with IV hydration and slow infusion. Probably in May. I will report back on this. After a month I am told to let me doc know what dose I want at the 3 month mark. I read, and my doc agrees, that a half dose is as effective as the full 5mg so I am aiming for that as a total for the first year. I have GERD and cannot tolerate alendronate but if you can, then the advice of @artscaping is the best!

@windyshores how has Reclast been for you? I will be transitioning off Tymlos probably the beginning of April so I’m trying to decide what is next. My doc wants me to take Prolia.

My friend was able to get financial assistance from Forteo for her Forteo Rx. They paid part of it. She couldn't get financial assistance for Tymlos, but they are pretty much the same drug except with Tymlos, you can customize the dose if needed so you take less than the full dose. I am currently taking Tymlos, for about 1.5 years and am doing very well on it. My insurance company pays for it other than a $50 copay.
I am surprised to see the # of patients on this forum whose doctors didn't do baseline markers of CTX, P1NP and other testing. My endocrinologist is thankfully very aware of their importance and tests me fairly frequently.
I will be going off Tymlos in a few months. My doctor has talked about my going on Prolia next, but with all the comments here, which I am very grateful for, along with information from Dr. Keith McCormick, I am very wary about the rebound effect from Prolia. So I will probably opt for Reclast to lock in my gains from Tymlos.

@ginhu Are you taking the full Tymlos dose (80 mcg)?? I am very concerned about that and cannot get a straight answer from the manufacturer who keeps stating that 80 mcg is the standard dose. So a 105 lb woman gets the same dose as a 150 lb woman or a man? That seems wrong given that the lab rats that developed bone cancer did so more quickly dependent upon the dose amount they received. They insist that the rats received the meds over their full lifespan (mostly 2 years) but I feel that is secondary to the dose amount administered. At the highest dose, 87% of the male rats developed bone cancer. So when I ask the manufacturer if it is okay to receive a lesser dose since I am only 105 lbs at 5'5", they do not provide a meaningful answer.

I have a recent post with my regimen--Tymlos, then Evenity. My provider is recommending Reclast after that.

I did well on Tymlos, will have another DEXA scan in June to see whether Evenity did well also.
