Double Mastectomy-Considering DIEP flap~Timing and Recovery Questions

I was diagnosed with Ductal Carcinoma in Situ (DCIS) ER+, Nuclear grade 3 in my left breast. My local physician was so nonchalant about the diagnosis and surgical options that I chose to have a second opinion at Mayo.

At first, I chose a lumpectomy, and it was scheduled, but after my surgeon conferred with a vascular specialist, it was determined that my underlying clotting risk due to Fibromuscular Dysplasia and history of Carotid Artery Dissection meant I couldn't take the Tamoxifen required after surgery and radiation.

I have decided to do a bilateral mastectomy with reconstruction because there were some calcifications in the right breast as well that would require 6-month mammogram checkups due to their suspicious nature.

I am leaning towards a DIEP flap, but I am unsure of the timing of reconstruction (would they do this the same time as mastectomy?) and the recovery of a DIEP flap. My sister had the same diagnosis as me and she had a TRAM flap (not at mayo, and five years ago) and had a lot of issues.

I can't see the plastic surgeon until a few days before the surgery. The nurse for the breast surgeon said they can't do the reconstruction the same day, but quite a few people on Mayo Connect said it was done the same day. She said tissue expanders would be placed and reconstruction done later, but my understanding from the plastic surgery educator visit was that tissue expanders wouldn't be needed. I am very confused and it's hard for me to wait to talk with the plastic surgeon for the answers, especially since it will be so close to the surgery date. I'm a planner, and I have the patience of a carrot. I would like to know your experience.

Does anyone have experience with double mastectomy and DIEP flap to share your experience? Are implants a better choice? What does the timing look like? If there is a delay between mastectomy and Diep flap, did you have tissue expanders?
