I can't tell if my ear problems are severe or if I'm crazy.

I have had ear infections all my life and currently wear hearing aids. You are not over reacting at all. As @ dragonfire stated this sounds like an infection in the middle ear that is not getting better. Viral infections can cause skin rashes.
Hopefully this is a bacterial infection which can be easier to treat. You need an emergency ENT appointment and antibiotics so see if someone can get that for you. Leaving it untreated long term could cause hearing loss.
Do NOT put anything into the ear…unprescribed drops are a no no. You can contaminate the ear so easily . Stay away from Qtips. The heating pad is a great idea. As far as an emergency ENT appointment, if you need to exaggerate the pain..go ahead and do that and see if you can be seen earlier.
Advil or or any ibuprofen is an anti inflammatory so I would suggest taking that instead of Tylenol, if you can.

I agree with everything @imallers stated don't put anything into the ear. Qtips can make things worse. Go to an ENT and if not satisfied try others. I went to 6 before finding one that could help. Don't wait ears need attention before anything happens and infections need medication to clear up.
