What are Short-term ADT's long-term side effects?

Posted by quaddick @quaddick, Oct 13 11:07am

My RO suggested 4 weeks of RT combined with 6 months of ADT. I've heard very bad things about ADT's side-effects. Are the side effects for short-term ADT only temporary?

@brianjarvis My experience was substantially the same, except I had 6 months of Orgovyx, but also 28 proton sessions. Very few side effects. Biggest was no libido, not sure about ED since I had no libido, but no ED now 3 months post Orgovyx. My testosterone went from 6 to 110 one month after stopping ADT. I had a few "warm" flashes but that was about the only other SE. I also ran and lifted throughout the treatment, and as with Brian it was something I had been doing regularly before so it wasn't hard to keep it up. All men have different experiences but from the readingI've done, and experiences of men on this forum, exercise seems to the key to reducing SE.

@daveinflorida Yes, libido takes a hit due to low T. My medical oncologist advised me that though the libido will be gone and the “want to” won’t be there, to “do it” anyway. I was told that it’s a “use it or lose it” scenario, and it’s best to keep the blood flowing down there for better recovery. Libido eventually returned to normal after stopping the Eligard when my testosterone levels returned to normal.

@daveinflorida - hey Dave - I'm currently on my 7th month of orgavux, I finished 28radition sessions mid August. My doctor wants 3yrs. ADT. My situation was gleason 8 with seminal vesicles also infected. My 1st psa came < 0.1 . Just wondering your decision to only do 6months of ADT. I have been reading many studies suggesting intermentent ADT is okay with psa test regularly. Thxs - Gary - Sebastian Fl.

@garyhu NCCN guidelines call for 12-36 months of ADT with hormone therapy for high-risk (Gleason 8) prostate cancer (see attached chart) - on the short end if there are no other risk factors, on the long end if there are other risk factors.

@garyhu My MO suggested 6 months. I was GL 3+4 and think that is the standard recommendation for that Gleason level.

@brianjarvis My MO never said anything about "use it or lose it" and even though I didn't use it for 6 months it came pretty quickly once I finished the Orgovyx. Doesn't seem to be any lasting issues.

@daveinflorida
Thanks. Glad to hear it. I've heard Orgovyx has less side effects.

@quaddick
Orgovyx Doesn’t really have fewer side effects. It drops your testosterone to close to zero as it can get the same as every other ADT drug. The thing is if you are on a short term dosage, then the side effects don’t really accumulate like they do if you’re on it for an extended time.

I’ve had no problems with side effects as long as I did my HIIT run on a treadmill 2 or 3 times a week. On Lupron and Abriraterone for 24 months. Just finished three months ago.

@ghtomato thx, just started my ADT w/Lupron shot. Going for my 2 week check up this week. Wondering when to start radiation. Want to start before end of year, already hit my insurance deductible.

@mbambenek
They do usually want to wait a couple of months after ADT before they start radiation. That two months allows the drugs to shrink the tumor and make it easier to radiate.
