Lower abdomen, side, and lower back pain
Was diagnosed with 7 (3+4) prostate cancer in April, PSA is around 4.5, biopsy confirmed 1 of 12 cores in area of concern (70%, 4 being 10%). Had PSMA PET full body, CT with contrast, and pelvic x-rays and nothing shows spread. Had all sorts of other tests including Prostox, Polaris, and Artera. Everything came back lower grade, early caught, chance of spread very low, but I’ve had lower back, both sides, and lower abdomen pains for some time now. I’m evaluating treatment options, but every time I feel these pains I think the cancer has metastasized. Can anyone who has been through this explain to me if they’ve had any weird pains or feelings when the cancer metastasis, or do you not feel anything. Sorry if this is a stupid question. Thanks.

@brianjarvis Holly Cow, 11,700, 7,500, 3,800!! were those typos!!
Trust the science. It is easy to worry about every pain when facing a PCa diagnosis. I remember focusing on every ache and worrying about metastasis. It's been two years since my RARP and my PSA is undetectable. Good luck.
@ucfron Wing , need to be doing all the right things. With this lesion fairly tight to the exterior, I wouldn't delay a year or two. I would probably deal with this as soon as you can and after you've done your research. I ended up getting the operation at 57. The operation went well however the PSA did not bottom out . About two years ago I ended up getting salvage radiation at 0.14 PSA. My PSA hovers between 0.061 and about 0.035. These are very low numbers and they seem to hover around there. I feel good and I go hiking every day. Yes Vancouver island is a treasure. A lot of people don't realize it was its own colony and did not want to join up with British Columbia for the longest time. There is still rumours that the vote was rigged and Vancouver Island what's the town country. At that time under the watchful eye of Britain. Lots of resources on Vancouver Island. Where I go hiking I walk on cold beds of clean cool about half a kilometre thick. It pokes out of the Earth in certain spots. There are also some uranium sites and gold and rare earth metals. The wild rabbits are everywhere now and the dogs take off after them constantly. About five varieties of salmon are coming up stream now so the rivers and streams are full of them. Again the dogs are after those too . I reside here for half the year and normally have the year in Southeast Asia where I grew up as a foreigner kid. In the winter, I will go to Southeast Asia, however we don't get really harsh winters here on Vancouver island, on the East Coast we have a micro climate to keep things very warm because of the Japanese current. Also, the westerlies keep us quite warm as well . Little bit of Vanc Is stuff for ya ! 🙂 God Bless Sir and kept in the loop . yoru on the right track , keep close to Dr's advise . .
@ucfron I take this as good news . The scan showed zero evidence of metastatic processes. You always have to be aware that there is microcells circulating the body at all times however usually the immune system takes care of that. I think this is a very positive and happy report. James on Vancouver Island .
@ucfron Not typos. The guy who had the 11,700 PSA was given 3 years to live; he’s now at 5+ years and attends an online prostate cancer support group that meets every Wednesday evening.
When someone has VERY advanced prostate cancer questions, that’s the guy I refer them to.
@vancouverislandhiker I do plan on doing a treatment later this year, but I believe my lesion is more to the inner of my prostate which maybe why my Artera AI test came back saying I have very low probability of it metastifying over a 10 year period. It said like 1.5%.
When looking up the wording in my PSAM PET it says;
The left paramedian transitional zone of the prostate is located in the central part of the prostate, surrounding the proximal urethra. This zone is responsible for benign prostatic hyperplasia (BPH) and is situated adjacent to the peripheral zone.
The prostate is divided into several zones, each with distinct characteristics and functions. Understanding these zones helps in diagnosing and treating prostate-related conditions.
Prostate Zones
Zone Name Location Description Cancer Origin Percentage
Peripheral Zone (PZ) Surrounds the distal urethra, located posteriorly 70-80%
Central Zone (CZ) Surrounds the ejaculatory ducts, located centrally ~2.5%
Transitional Zone (TZ) Surrounds the proximal urethra, located centrally 10-20%
Anterior Fibromuscular Zone Located anteriorly, primarily composed of muscle and fibrous tissue Not applicable
Left Paramedian Transitional Zone
The left paramedian transitional zone is a specific area within the transitional zone of the prostate. It is located adjacent to the left side of the urethra, which runs through the prostate. This zone is significant because it is where benign prostatic hyperplasia (BPH) commonly occurs, leading to urinary symptoms.
Clinical Relevance
The transitional zone is responsible for the growth associated with BPH.
Approximately 10-20% of prostate cancers can originate in this zone, although it is less common than in the peripheral zone.
