I believe if I need such a specialist, the burden should not fall to me, the patient, especially as a VA patient and such a specialty is required. The have a lot more resources. But thank you for responding.

@bug3 That is an interesting point of view. On the other hand, who has a greater interest in making sure you get the right treatment than you? And given current staffing shortages and budget cuts at the VA, who there has the time and expertise to find the right doc for you?
As we often say in the MAC and Bronchiectasis group (rare conditions that few docs treat properly) it is in your best interest to be your own advocate.
