Which option to pick???

So, I saw my doc today. They said, even with exercise/supplements, that won't be enough to improve my bone density/fracture risk in any meaningful way. The meds that were suggested were reclast (which I understand that only prevents further damage but doesn't build back bone) OR Tymlos (which I would take daily for 2 years, not sure what happens after the 2 years). Anyways, anyone have experience with these drugs? Can I avoid them all together with supplements and exercise? Appreciate your comments, thanks.

gweniepooh, the zometa infusions given after cancer are usually more frequent in smaller doses. You won't be likely to have the same side effects as those of us taking Reclast (5mg zometa). You can ask for slower infusions giving the kidneys more time to process the drug. It is a good choice shouldn't affect spondylosis and may help protect from the damage sometime caused by RA and will protect your bones from any (unlikely) metastasis, while protecting you from any advancing osteopenia.
While there are no known drug interactions between the two. Using them together may increase the chance of experiencing flu like symptoms and/or bone pain. But your doctor will probably be adjusting the dose in that situation. Congratulation on your successful treatment and early catch.

i keep posting this story I saw on substack in response to different questions because I am so impressed. It explains why the Liftmor/onero program works -- look for a PT who is trained in it near you, and it may not be that near; i have to go 1.5 hours if i decide to do it - and why all the stuff we all have been doing , the vibration plates, weighted vest, etc, DON'T work. We have to lift a lot of weights and jump. He explains why. I have decided not to do meds and have also decided to concentrate my efforts in Onero type workouts and making sure I get adequate nutrition on all the important foods my body needs. I hope you have great gains and please keep us updated!!

I am taking Evenity but only for 6 months because most of the effectiveness is in the first 6 months. I will have a DEXA scan and then take Prolia infusion annually for 3 years

I have a question about why your doctor would do that. Evenity builds bone the first 6 months and then suppresses breakup of bone the last 6 months from what I understand. Prolia also suppresses bone breakdown, so I don’t understand why not finish the last 6 months of Evenity??
Just make sure your doctor does a baseline test of your bone turnover markers (CTX for bone breakdown, P1NP for bone building), and then test again 3-4 months after you start taking your meds to make sure they are working. There is nothing worse than taking a year of medication and then finding out they are not working! I read somewhere that certain osteoporosis meds do not work on 5% of the population.

Do you have any underlying health issues causing your bone loss?? Address those first.

To answer your question, yes, you CAN slow down or improve your bone mineral density by doing certain exercises (weight bearing, check out the LiftMoor/Onero exercises by Dr Sheri Betz), eating the proper diet, taking supplements, etc BUT YOU HAVE TO WORK AT IT!! It is much easier to pop a pill/do an injection. Also, each medication has risks. You would have to weigh the risks vs rewards of those medications.

Educate yourself about osteoporosis. Don’t rely solely on your doctor’s advice because many of them are not very well versed about Osteoporosis. I have found the following on youtube to be very informative; Dr Doug Lucas, Margie Bissinger, Dr Susan Ott, Margaret Martin, OsteoBoston

Don’t forget, your DExA score is just one part of the picture. It measures bone quantity NOT bone quality. Also look at your FRAX score. Is it good? Is your overall health good? How is your balance? You can work on that by doing certain yoga exercises and tai chi.

I have not been feeling well on Evenity so 7 months is what I will tolerate.
I am scheduled for a DEXA in December but so far, it is out of pocket for me because Medicare won't cover it. My doctor basically threw it back to me because they don't have the time to appeal to Medicare on my behalf 😒.

Any thoughts?? (Other than my doctor is not my favorite)

clowny, I like the idea of taking the first best months of Evenity. but why Prolia (other than that the manufacturer recommends you stay on their medications). Are you afraid of Prolia rebound after the three years.

It's a tough decision. Every drug has a possible negative effect and there is no way to know how you will react. You make your best choice and hope for the best! I am currently using Prolia and praying I don't have negative outcomes because bisphoshphonates don't work for me. I would say used Prolia only after you have exhausted other options.

I am sorry to hear that you haven’t been feeling well on Evenity. I hope that being on it has increased your bone mineral density and you can lock in those gains by going on Prolia or ReClast.
