Testosterone levels are rising after cessation of ADT

I am being treated for high risk prostate cancer . Completed radiation ,ADT AND Casadex for 2 years. I have NOT taken ADT or Casadex FOR 12 months …my psa still remains undetected. However I am noticing my testosterone levels moving closer to the normal range….is this a good sign?

Here's my clinical history. In both cases, T rose to 300+ after three months and 400+ after six months after completing treatment.

In the first case, the PFS was roughly five years. In the 2nd, we are at 17 months, and PSA is stable at .03.

I am not naive enough to believe I am "cured...!" For high risk Advanced PCa I believe medical research has brought about ways to manage through imaging and treatment, but cure, no.

So, enjoy your T, continue to actively monitor through labs and consults. Think about and discuss with your medical team what clinical criteria would warrant going back on treatment and until then...

My testosterone is rising like a snail climbing an aquarium. So my arms look like prunes. It’s been since january2025 I ve been off ADT drugs. Latest check- 20 is my level, when it should be 193 at my age of 76.

I am stage 4a, grade 5, have had surgery, radiation and am currently 18 months into ADT therapy.
My PSA is undetectable still, my but testerone has jumped up to normal levels while still undergoing ADT injestions.
Wondering if I am now jumping into castration resistance phase.

Something doesn’t sound right. I’ve NEVER heard of anyone taking ADT that didn’t lower the testosterone. And I’ve been in this battle for 10 years now. Definitely do a redo on your lab work.

I was on Lupron(adt) for 2 years... I also was on casadex for 2 years. I had radiation therapy first.
I did not have my prostate removed.
Gradually after 18 months of no ADT my testosterone levels returned to normal.
My psa has risen slowly to .09 but my oncologist says that is to be expected as testosterone levels return .
I have been assured that I have no biochemical recurrence at this time.
I am 4 years after my initial diagnosis . Hope this helps

Sorry Mike, but you stated that your ‘ADT is continuous’ in one post and then ‘on ADT now in month 18’ in another…??
But now you say you’ve been OFF ADT for 18 months…can you see my confusion?
But no matter, I am glad that after you being off ADT for 18 months your T is back to normal and PSA is staying low - the BEST combo there is!

I’m no expert but how can your T be approaching normal levels if you’ve been on ADT for 18 months? What does your Doc say?

I had the same result after 13 months of Lupron. I was also on darolutamide, which I've continued alone for subsequently 18 months. I assume the darolutamide is blocking utilization of T. My PSA is < 0.1.

Nubeqa (darolutamide) works in the presence of significant testosterone because it is an androgen receptor (AR) inhibitor, not a testosterone-reducing drug. It blocks testosterone's ability to stimulate prostate cancer cell growth by binding to the androgen receptor and preventing testosterone from attaching.

There are quite a few people in the Weekly ancan.org Advanced prostate cancer meetings that are only on Nubeqa. Most of them are older, and they really like the results since ADT can be difficult.

Can you send me a link to that weekly meeting. I too am doing Nubeqa mono therapy and would like to communicate with others doing the same. Stopped Lupron 6 months ago(had last 3 month shot 9 months ago).

Ancan.Org is the organization that does these weekly meetings. The next one is Tuesday the 23rd at 3pm pacific time. Get there 10 minutes early, That will make you first when they start talking to people. You should mention that I recommended you come to the meeting.

You can sign up at that website for information. They will send you a newsletter, Though it just came out today so you may not get this weeks newsletter.
