How bad is a spine T-score of -4 ?
Is it too bad?
Is there a chance for "turning back" from such score?
Has someone actually improved it?
I feel devastated to see this score, Given my age, I don't want to live all my life in fear that I will fracture. Just asking for your opinion/experiences.

Hi @mfanslow, Could you explain in what way a T-score declines as one ages? Are you saying that the T-score of a 50 year old would be different than the T-score of a 80 year old with the exact same bone density?
I did not think it worked that way. I thought the T-scores would be the same if the bone density is the same. If this is not so please provide a reference. Or perhaps I misunderstand you completely?

Getting TBS score would help so much to see the quality of my bones. I did fall this year; backwards, from my dogs running towards me, and nothing broken!!
Hi, those numbers are L1-L4. I would like to know TBS as well but the software isn’t part of the Hologic scanner used on me. My understanding is that REMS can be done as often as six months or certainly annually. DEXA shows more gradual change and I suppose that’s why insurance will generally cover only every two years. To understand more about REMs, you might search for Dr Kim Zambito and REMS. She writes a blog and has participated in videos to explain how DEXA and REMS are apples and oranges and can be used together. She’s an ortho surgeon. Dr Doug Lucas in last three weeks has also produced a video on REMS and DEXA. You may have to view / read more than once to understand fully (at least, I did). 😀

I saw another video by these two on a suggestion here and it was very informative too..I’m so glad u’ve shared this one.
What I have learned is that not all bone drugs work. I am one after a year on Prolia with basically no gains and no you don’t have to be on Prolia for life.

My T score in my spine was -4.6 then went on Evenity for one year with improvement to -3.3 then went on Prolia for a year with basically no change -3.2. Now I will go on reclast.

1 ReactionSophie, I was diagnosed with severe osteoporosis a year ago. My T score for my spine was -3.6. After 12 months of Evenity my T score was -3.5. I did everything possible from a high protein and calcium diet to walking 3 to 5 miles a day, lifting free weights and physical therapy twice a week for one whole year. I was told by my doctor that I’m in the 5% group where Evenity did not work for me. Not sure why, it could have been my own antibodies neutralized the drug, my thyroid disease or the osteoporosis was so severe when diagnosed that is was too late to turn it around. I’m on Prolia now and still maintaining my same routine of diet, exercise and prayer in hope of a lower score come 2026. I pray that you will have a better outcome than me.

1 ReactionIt amazes me how some drugs work for others and then not for some. I did really well on Evenity went from -4.6 to -3.3. Then went on Prolia and basically had nothing went to -3.2. I was also on hormones during the time I was on Evenity and maybe that helped also.
Prolia is used to preserve bone density (anti-resorptive) while Evenity is used to increase bone density (anabolic) so that's probably why you didn't see a significant gain while on Prolia. But it's good that it also prevented bone loss after your Evenity treatment.
Why did you stop HRT? Continuing on it if you can would also help you preserve the bone you built from Evenity.
