I am 83 years old with a PSA of 6.5. Will have a prostate biopsy soon.

Would be interested to know what kind of treatment options I have available, depending of course on the biopsy results. I am in reasonable good health, with high blood pressure being treated. How limited are my options based on my age of 83?

EBRT usually implies electromagnetic radiation (photons). Protons are a different particle and the behavior is different. Can be targeted closely and does not penetrate far beyond the target unlike conventional EBRT.

My original answer said electrons instead of photons. I blame Lupron for the brain fog.

My Urologist was quite upset when another MD ordered a PSA that had a result of 14.6. The results found their way to his desk and he called me in saying that now he 'had' to do something about it. I was 88 at the time.
A few months later, at 89, I had an MRI, biopsy, and PSMA, the latter indicating that the cancer was contained in the prostate. My Gleason was 4 + 3, as the MRI, and follow-up biopsy confirmed the presence of cancer.
Because of my age, the Urologist recommended that I do an 18 month course of ADT which included 1 month of Casodex tablets, and my first injection of Lupron D. I am on the time release 3-month Injection cycle, and at age 90, had my 3rd one on Valentine's Day.
My PSA has dropped to .03 after 6 months of treatment, and was .05 after the first 3 months. The MD wants it to be undetectable.
Side effects were hard to take during the first 2.5 months with hot flashes, muscle weakness, weight gain, and no energy/fatigue. The only remaining one is a 6:30a hot flash, the others having dissipated.
I do my best to stay active walking 2-3 miles each morning and watching my diet.
Other that the Urologist's disdain, I am pleased with how treatment is progress thus far.

After SBRT radiation I was on Orgovyx for 12 months. It has been 3 1/2 months since I stopped Orgovyx.
I just got my Testosterone result yesterday. It was only 65. Waiting for Ultra sensitive PSA. It was 0.02 before I stopped the treatment.
How is your Testosterone recovery after you stopped the treatment. I was told the older we are the less chance to get any T recovery!

I will be seeing my urologist on July 30, and it remains to be seen what he will suggest considering my age. He told me nothing about the value of exercise or how to deal with shrinkage when I first started treatment. Joining this group as well as a local support group has been a Godsend.

It has been a slow recovery from the ADT, as although the hot flashes stopped quickly and libido has returned, I am still lacking in energy, and experience cramping and soreness in both legs. Nevertheless, I keep going and hoping for the best. I wish likewise for you!

A drug like Nubeqa (Darolutamide) might be what would work.

I’m 77 and I’ve been on it for two years and it’s the first drug in 15 years that has kept my PSA undetectable for 20 months. Previously, I got one month undetectable. I know a lot of people that have been on it as their only drug. It has very few side effects for most people. It works well, even if you have a testosterone level. I quit ADT For eight months while I was on it and My PSA stayed undetectable. I know a lot of other people all ages that are using it as their only drug. You could come to an advanced prostate cancer weekly meeting at Ancan.org And they can give you a lot of cases where it could work for you too.

There’s four non ADT drugs that you can choose from. Nubeqa is much better in side effects than the other 3.

Ask your doctor about it.

Amazing! Have seen it mentioned on here, but don't recall it being validated so strongly!! I will certainly talk with my urologist about it on the 30th if he wants me to continue with some form of ADT.

You had a quick recovery of Testosterone but still low on energy? What was your Testosterone level before the treatment. Mine was 328 at age 88.
Is your urologist also an oncologist? As jefmac on this forum says we should see a GU Oncologist for treatment with drugs.

Maybe so, as I have always had low T( not sure if genetic), and prior to cancer diagnosis, hovered in mid to low 300s. At one point, I was placed on a precription booster, and my levels dropped, so go figure, and was taken off of it never to return. Having low energy is depressing.

My urologist is not also an oncologist. And, when I asked about a referral, was told that oncology was only for those on chemo, radiation or treatments other than ADT. I haven't fought it.

Time to find a doctor that does more than urology. Your doctor wants you to keep him on because he makes so much money having you are treated by him.. Doesn’t matter that he is an Oncologist he is not informed enough to handle More than a basic low-grade prostate cancer that just requires surgery and nothing else.

You want a center of excellence or a Genito urinary oncologist handling your case. They will know how to handle a case for someone in their 90’s. Your doctor sure doesn’t since he gave you Lupron at that age.

Don’t ask for a referral, call a center of excellent and get an appointment. Just stop working with the doctor that doesn’t have the knowledge you need.

WOW! What a fantastic video! I wish I had seen it before starting treatment, but glad that I ended ADT aftet 4 injections rather than continue with the two more that my urologist had recommended!
I have a follow-up appointment on July 30, and with a PSA of 2.5, I plan to tell him "no more treatment"!
I will try to locate a Center of Excellence (?) in my area.

See if you can get Nubeqa. It works all by itself and can really control cancer with very few side effects. I’ve been on it for two years and haven’t had any issues at all with side effects from it. I know a lot of people who Participate in the weekly advanced prostate cancer online meetings at Ancan.org That are on Nubeqa As their only drug. It works even with testosterone.

I do know of one case where the person had a side effect problem and couldn’t handle the drug. I know over A dozen people on it and really pleased with the results.

Stopping treatment completely could lead to a painful death. My father died from PC and it was extremely painful for him in the end.

If your doctor doesn’t know about it and won’t prescribe it, find yourself a different doctor because a knowledgeable Genito urinary oncologist will know that it will work for you.
