Any experience w MRI-guided stereotactic adaptive radiotherapy SMART
Hello, I am 67 years old, very fit and sexually sctive and have been on active surveillance since age 62. Although my PSA hasn’t changed and has been steady at 4-5 over the period, my latest biopsy showed 80% of the cores had cancer. Decipher showed high risk and Polaris showed low risk. Concurrence of urologists at first second and third opinions at major facilities was to pursue whole gland treatment. I ran across MRI – LINAC or MRI guided SBRT adaptive therapy. It seems the machines and the technology are scarce and only at centers of excellence. However, cure outcomes and limited side effects are excellent. Anyone have any thoughts, experiences, input vs RARP?

I had a consultation with a Moffit RO in May 2025 and he indicated that the facility will stop using the mridian viewray on may 30,2025. He said the elekta unity (mri-linac)has been ordered. I was offered HDR brachytherapy.
Additionally, I had a consultation with an RO at Orlando Cancer Center in early June 2025.
She indicated that the Mridian Viewray machine has been down since April 2025 and that the elekta unity has not been budget approved. I was offered sbrt with Ethos. Subsequently, I received a portal message from her NP indicating that the mridian mri-linac may be back online 8/1/25 but will be limited to liver and pancreas cancer.
also, note both of these facilities continue to advertise mri linac treatment.
Wow good info thanks. Advent recommended HDR Brachytheropy to me as well. Was looking for alternative. Does anyone know if Mayo in Jacksonville has MRI Guided SBRT?
