Do you mean your eardrums are perforated? If si, topical meds usually don't work so you might need to see a doc for oral meds.
Please don't put anything in the ear canals unless directed your doc. If it's persistent, best to see an ENT.

I would get off the terbinafine since it’s not helping and it can be hard on the liver. Older posting on this subject suggests that Gentian violet might help. I can only suggest that you keep your ears as dry as possible. Use a blow dryer, gently, on your ears after showering. They sell blow dryers just for ears. When I go to the salon I put two cotton balls coated with Vaseline into my ears before she washes it. You could do this at home. Swab out your ears during the day if they feel moist..especially if you wear hearing aids.
Keeping the ear canal dry is the key and go for regular ENT checkups. I have only ever had that one time with the fungal infection.
My ears have been so good for the past several years that my ENT guy said that there is no need for me to see him as often so we settled on twice a year for checkups and cleaning. I am diligent about keeping moisture out of them.
