Pathology report is in 😮‍💨

I was diagnosed with Gleason 4+4=8 in late December 2024. Had RARP on 06/16/25. PSMA was negative and Decipher 0.2. My last PSA was 5.5.

Pathology:
Negative surgical margins
Gleason 3+4=7
Stage T2c
No SVI
No LNI
Cancer involvement 11-20%
No adverse features such as Cribriform, IDC, or pattern 5.

Hugely relieved. Surprised by the downgrade from 4+4 to 3+4 (After Gleason 8 diagnosis I went into a mental tailspin of anxiety and depression).

Only fly in the ointment is that the stiches did not heal and that I will have to wear the catheter for another 10 days. 🤦

TOPF - Nice pathology report with a down grade in Gleason IS solid good news . Any other symptoms fro you surgery ? What is your age and what was the PSA going into your surgery ? Stitches healed now - should be ... God Bless Sir . James on Vancouver Island .

No symptoms thus far. But it is still early days. I hope the catheter issue resolves. I recenrly turned 52. My ladt PSA pre-biopsy was 5.5, before it had fluctuated around 4.0 for ten months.

Thanks! I did get a second opinion on the biopsy with Hopkins-same result.

The issue is that a biopsy is too small a sample size to to yoeld a reliable estimate. And in order to err on the side of caution, the highest grade core is reported as the overall Gleason score. The parhology Gleason score, by my understanding, is based on the two prevalent patterns in the entire prostate specimen.

Just AS (PSA test every 6 months) as PSME showed it was contained to the prostate - they also took some lymph nodes and they were negative.

Luckily, same for my lung cancer but that's another story 🙂

I was targeted at 4+3 with the biopsy . After operation they do a detailed look of Prostate and I can in at 3+4 . It was a nice surprise .... like yours ! Keep up the good work ! God Bless . James
