On DVRD for Multiple Myeloma, bone marrow biopsy percentage went up.
My husband was diagnosed with Multiple Myeloma 12/24. His plasma cells were 20% on first bone marrow biopsy. Repeat biopsy 5/25 and the plasma cells went up to 30%. Anyone else have a similar situation?
They told us a few reasons for this could be-
-They hit a “hot spot” in his marrow with an excessively high amount (Myeloma is patchy/clustery not uniform).
-His bone marrow is being slow to respond.
-An “overzealous” pathology reading.
-Or this immunotherapy is just not working like they want.
They are postponing stem cell transplant for him to receive possibly 2 more cycles of DVRD. All the rest of his labs (M spike, IGG, light chains) are almost all within normal now. First PET showed 1 lytic lesion. Recent PET = no new lesions and original lesion is stable and no longer hypermetabolic.

I appreciate your question. I don’t know the answer; I like seeing other’s numbers because I feel in a solitary vacuum with mine, no awareness of what numbers look like for others. I raised my eyebrows with the “overzealous” path report part. It sounds like the doc is in denial? Can you get another doctor involved? Is this treatment at a cancer center? Do a bunch if docs round table and discuss all the cases to plan the best course of treatment? That’s the gold standard but some clinics have few specialists and can’t do this. I’m going to a cancer center and feel good that they discuss my case in a formal setting. It’s like having 4 or 5 brains agreeing instead of one who might not see the full picture of options. Is he going to a myeloma specialist? I look forward to reading what others suggest or answer your question. I too would be concerned if the numbers went from 20to 30%. That is a mathematically significant jump, imo.
