@crazyelf I do believe that this not paying attention to medications and interactions, happens within the medical community as a whole, not just the VA system!

The best way, in my experiences, is to be as much of an advocate as we can for ourselves. In whatever form that might take. For me, it has meant sometimes questioning a prescription, discussion with pharmacist to get their opinion, and sometimes having a frank discussion with a doctor.
