PSA question
So, I began ADT (Eligard injection) 9 weeks ago. At the 6 week mark my PSA dropped from 12 to 0.6. I liked that. Yesterday (9-weeks in) I had another PSA test. It was 0.7. I didn't like that bump. The oncologist told me with confidence at the 6 week mark that my PSA would be negligible by June. Is this a concern?
No radiation yet. Just ADT. Testosterone is unchanged during this period at 0.2.

From 0.6 to 0.7 is just normal test-to-test variation; I think you can conclude that while your PSA hasn't become undetectable yet, it's fallen to a very low level and is holding steady (my PSA took a bit over 3 months to reach undetectable levels on ADT+Apalutamide).

You have had no treatment besides ADT, so your cancer is active. If you were to have had radiation, it would not be unusual for your PSA to not drop to undetectable even after ADT, but below .2 is acceptable, they usually don’t do anything, unless the PSA rises 2 above the lowest it ever hits. If you still have untreated prostate cancer then your PSA can definitely bounce around.
I see you are going to see a radiation oncologist in June. You need to find out how large the cribriform is, If over .25 mm then it is truly a problem. SBRT Radiation alone Can’t treat it, may need to have brachytherapy if surgery isn’t done. Just some things to discuss with the oncologist.
Thanks Jeff. I'm thinking EBRT will be suggested (28 sessions). Also, since I had TURP years ago there apparently isn't enough prostate tissue to qualify for Brachy. Surgery at my age ain't happening.
All my biopsy report says is 'cribriform is present' as well as PI is present.
You mentioned that your “Testosterone is unchanged during this period at 0.2.” What units is that 0.2 in?
I became ill with high fever along with urination issues. Had urine and blood test. I had a UTI and PSA of 7.8, 7 months earlier PSA was .6 Dr sent me for ultrasound and showed BPH. Dr. Put me on Floemax and went to urologist. He said PSA was nothing to worry about that it was due to UTI. He upped Floemax to twice a day and set follow up for 3 months. I have a follow up with Dr in 2 weeks and am going to ask for another PSA which will be about 6 weeks since UTI. Urination frequency and flow has not improved yet 3 rd week of Floemax and 4th day of double dose. Any insights appreciated.
You could Request an MRI to see if there is anything in the prostate. Has The doctor has done a manual check to see if your prostate has any noticeable irregularities, MRI is better.
If your PSA has dropped around half or less then you could wait to do anything for the next PSA test. If it is only BPH it should settle way down.
You are not mentioning that you are taking antibiotics, they are essential if you have a UTI causing BPH. You are on an antibiotic, right?
I was on antibiotics for about 12 days so no more UTI but has blood test a few days after finishing antibiotics. Do you or anyone else know if a CT scan would show any irregularities of prostate? I had one just after UTI for nodule in lungs.
Also haven’t had a digital check in about 2 yrs but had a colonoscopy less than a year ago and I believe they check prostate while doing colonoscopy.
You really need an MRI to see what’s going on in the prostate. They come up with PIRAD scores for any tumors/growths found.
It's nanomoles (nmol/L). Should equate to ~ 58 ng/L
