Hi Buskydog,
What an uncomfortable situation to have to live with! And for all those years! How awful! I did some research and found the following suggestions that may help somewhat:

* Provide support to the scrotum by wearing an athletic supporter.
* Apply ice (wrapped in a cloth) to the scrotum.
* Take warm baths if there are signs of swelling.
* While lying down, place a rolled towel under your scrotum.
However you may have already tried these.

Also the following foods are supposed to help this situation:
* Omega 3 rich foods such as beef, salmon.
* Leafy green vegetables
* Berries
* Turmeric - a seasoning.
Have you talked to your doctor about seeing a specialist? If he doesn't help you, then just find a specialist and try and make an appointment.
Mary Ruth is right. Pray and read your Bible. Ask God for help with this. Just talk to him like he's your best friend and he is! He will be there for you. God has helped me and my family many times with pain. I'll say a prayer for you too.
I wish you the best!

Are you able to self refer? Sounds like you have done some research on physicians who successfully treat this condition.
