uACR is high, eGFR is normal: what are next steps?

Posted by John Taylor @jdtay87, 3 days ago

Like the title says. Any uACR over 300 is bad, obviously, 1395 is just freaky deaky. My eGFR is over 100 though, still healthy. My doctor has yet to review these results since they came in Friday night. Do you think he'll want a kidney biopsy, or are there other tests that can be done first?

This may be unrelated, but my a1c dropped from 6.8% to 5.5%. Imagine that. "Good news, you no longer have diabetes! PS you've probably developed chronic kidney disease." It's like healthcare whack-a-mole, knock one down and another one pops up.

@jdtay87 Hi John. You are posting a discussion in the kidney / bladder group and you mention the A1C test; correct? I take that to mean that you have both diabetes and kidney issues. Did you know that diabetes and hypertension (high blood pressure) are the two most common causes of kidney problems? I am unsure of what uACR is. Could you enlighten me? I have difficulty with acronyms and abbreviations but I’m VERY familiar with A1C and eGFR. Thank you for the “whack-a-mole” reference! That is truth!

uACR is urine albumin to creatinine ratio. Normal is under 30, elevated is 30 to 300, anything over 300 is severe. Having 1395 is really scaring me. I do often have foamy urine and swollen ankles and feet, which also point to kidney damage. I'm not sure why my eGFR is still normal, but according to a "kidney heat map" chart I saw, having a severe uACR with a normal eGFR still puts me in the orange or second highest category for possible kidney disease.

I called my doctor's office this morning to let them know about the bad uACR result, and they said I would probably get a call today about their recommendation. Well, it's after 5PM now, and the place is closed, and according to the online portal my doctor still hasn't reviewed the result yet. It occurs to me that he might not even start reviewing labs until after the place closes, since he sees patients while it's open. Just trying to be patient here.
