Suggestions please—lower back pain interrupting sleep
I’m hoping for suggestions from fellow sufferers who can’t sleep or keep waking up from lower back pain caused by arthritis. I can’t take nsaids, and Tylenol doesn’t help at all. Tried otc sleep aids that don’t work. Will be having hip replacement soon and doc says that may help with back pain but seems contrary to his diagnosis of arthritis in lower back. Stretching and chiropractic have not helped. I have an adjustable bed frame and use that to change position but that’s not helping any more. I use pillows under or between legs to relieve pressure but that lasts only a minute or two. Just bought some Salon Pas to try on lower back and hoping I’ll get some decent sleep. I wake up at least 6-7 times a night in pain and have a hard time getting back to sleep. Thanks in advance for any suggestions you might have.

Has anyone had pudenal neuralgia? Burning in pelvic nerve? Especially in female area? I also have low back pain and right hip pain. TY
@reneerenee
Did you have an accident or injury before the pain started? Did you have any pregnancies or complications?
Did you have lumbar/sacrum spine and pelvis/hip MRIs?
Do you have compression of spinal cord/nerve roots/nerves? Have you seen an orthopedic spine and hip specialist?
First recommendation is to get a referral to a pain management doctor or a physiatrist as cortisone injections to bring down inflammation might be helpful. Second suggestion is to use a pillow under your knees if you sleep on your back or between your knees if you sleep on your side. It might help relieve stress on your spine.
I wish you good luck as back pain is so controlling of your enjoyment of life.

Have you tried sleeping on a heating pad?
I have not. Guess it couldn’t hurt if on low.
I like a number 2 or 3 setting.
My pain in SI started after driviving for 4 hrs or so with right leg way extended. This was in 1983. Car had bench seat. Husband was asleep. He had big job interview and needed a nap. I didn’t want to awaken him. Got out of car with hip pain and sciatica. Back and hip never the same. I did have my daughter in 1980 by c section. I was also tboned by in car accident atvage 18. Never went to have anything checked because I had no pain or obvious injury but looking back it may have started spine/ hip issues. Broken tail bone in 7th grade. I had sciatica afterbthe long drive for 4 yrs constantly. Had 2nd baby in 1988. All pain cleared. Dr. said cortisone in
Body is extra high in pregnancy and probably healed it. 2 yrs later it returned after I arranged some furniture.. not heavy .. on deck. L5-S1 involved. Had all kinds of therapy. A machine called VAX-D helped with muscle relaxant and ibuprofen. 3 rd c section in 1994. Pain during. Got had. Took prozac and Valium for it. Pushed on. Had many mri’s/ scans. Called it muscle/ w L5S1 bulge. Used chiropractor therapy.. electric pads like giant tens/ ultrasound. Kept me going. Am now 69 and have what the Dr. called “ normal/ regular low spine deterioraton in all lumbar. I unfortunately had to have 2 very lengthy prolapse surgeries and hysterectomy in 2021. Severe scars and endometriosis/ fibroids. Had been in menopause 13 yrs. I now am trying a nerve block. Dr. Going in at tailbone area.
Yes. And ice .
I have a history, which could be similar to yours and I posted many times. I’ve had many surgeries that give me back the quality of life. I’m after. My degenerative condition means my back will always be in the process of deteriorating at 68. It started when I was in my 30s. The only thing you did not say is that you have a regular exercise routine in place that deals with your core. If you want a solution that doesn’t require destructive medicines or surgeries then that’s going to have to be your answer. And I’m talking every day or at least five days a week an hour and a half a day. Our body ages as we get older and it is our job or rather it becomes our job to have to work on it all the time. As a 68 year-old I take bioidentical hormones, bone medications, visit with the orthopedic surgeon, a couple times a year to stay on top of my degenerative condition, and I visit a MFR therapist twice a month who is amazing. From your description, I think you would benefit from a good MFR therapist and should find one in your area there’s a section here on it. At different stages of my life having been dealing with the same kind of issues you have. I have found that there’s different remedies for each stage of life. The steroid injections, relieve my pain 100% but they’re really damaging. The surgery changes your life forever and has to be left for when there’s no other options available. There are ablations, there are Botox and muscles, there is acupuncture, there is MFR and a variety of other things. Like bone medicines and bioidentical hormones. The one overarching thing that I think you’ll need to look at is an exercise regime that’s at least five days a week and I’m not talking about 20 minutes while yoga. I’m talking about an hour and a half of combined aerobic isometric resistance And whatever every day. People hate to hear the simple truth but that old exercise like the apple a day keeps the doctor away!
Well, I can tell you my circumstance that after 20 years of pain I had my hips replaced and that did a great deal, but it didn’t solve the problem. Having surgery at L3, L4, L5, solve the problem of the back pain. You should visit with a couple orthopedic surgeons. I used to live with a 2 ton gorilla on my back. That gorilla faded away after my back surgery and I got back to sleeping at night after 30 years of misery. Don’t just visit one orthopedic surgeon visit as many as you need to to find one that you trust. Make sure that you’ve done an hour and a half of exercise every day to strengthen your core. People don’t want to hear that exercises the answer, but I know people who have bad Bax who escape surgery by doing an hour and a half to three hours a day of exercise. I’m sorry but I’d rather have the surgery than do that 😊. In my case, the pain never entirely went away because my coxes or tailbone is damaged and nobody wants to touch that so I take light dose Percocet every day to manage the tailbone pain that is just “there“ and distracts me from the daily aspects of living. I’ve accepted that over 30 years
